An exciting opportunity has arisen for a part time (25 hours per week) Band 6 Community Diabetes Nurse to work with our Integrated Diabetes Nursing service covering Basildon, Billericay, Brentwood, Wickford and Thurrock areas. We are looking for an enthusiastic and motivated individual, with excellent communication skills, who is able to work autonomously, as part of the team and to be an integral member of the wider Diabetes teams in supporting people managing the complexities of diabetes management. This role requires facilitating patient structured education, having a sound knowledge of the principles of diabetes management and care, and a keen interest in professional development & advancing with technology. Therefore, experience of teaching and good IT skills are essential. It also requires a post graduate qualification in Diabetes or minimum 5 years equivalent experience.

The ability to travel effectively across the boroughs is necessary.

Main duties of the job Detailed job description and main responsibilities The post holder will need to have a clear understanding of the challenges faced by primary and secondary care in meeting the recommendations for the development of heart failure services, such as those contained in the National Services Framework for Diabetes. To undertake a holistic, unified assessment process consistent with the approach to Single Assessment in partnership with all agencies, to ensure that acute admission is prevented where possible and that this approach identifies and delivers services to meet and sustain patients needs. To keep accurate up to date records of patients defined within the case finding process and communicate timely and informative written reports and summaries for other agencies e.g. GPs, Consultants, MDTs and Social Services. To establish robust communication networks with patients, their carers, other members of the multi-disciplinary team as well as other agencies, and the whole health economy involved in the care of the patients. Job description Job responsibilities

Main duties of the job The post holder will be responsible for providing; supporting and developing the delivery of high quality; evidence-based Diabetes clinical practice throughout the local community, in health centre clinics and patients’ homes. The successful applicant will be required to work closely with the lead Diabetes consultant, GPs, nursing colleagues and other members of the multidisciplinary team. Starting with NELFT NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment.

As part of the process, new starters will also have the opportunity to meet the executive team, senior managers and attend a number of dropin sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

Probationary Period This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

High Cost Area Supplement – Fringe Zone This post attracts a High Cost Area Supplement (Fringe Zone), which covers areas just outside the M25, including parts of Surrey, Hertfordshire, Essex, and Kent.

The Fringe zone weighting is 5% of the basic salary, with a minimum payment of £1,346 and a maximum of £2,270 per annum, pro rata for parttime staff.
