About this role

Job summary

The post holder will be responsible for leading the implementation of Fall Prevention Service within Havering. The post requires a car driver and be able to drive to BHR Borough.

Working collaboratively with all staff groups the post holder will use evidence to support the implementation of falls prevention care plans and processes to efficiency, improve patient assessments, access to treatments and improve the patient experience. The post holder will develop and lead the implementation of a strategy to ensure falls prevention can be embedded within the area. This will entail working with staff across all agencies and professions throughout Havering. The post holder will develop effective relationships, which will support the delivery of sustainable change.

Be experienced in the delivery of health care with an understanding of challenges of falls Management. Have experience of leading and participating in the delivery of change management projects. Understand of quality and service improvement techniques. Knowledge of falls prevention strategies. Exercise a high degree of professional autonomy. Ability to make recommendations based on national guidelines on falls and bone health initiatives and research.

Main duties of the job The successful applicant will thrive in a busy and demanding role, demonstrating strong clinical reasoning and excellent communication skills to ensure patients receive timely, holistic assessments.

They will manage a specialist caseload, including individuals with complex needs, undertaking comprehensive assessments, delivering and reviewing therapy interventions, developing person-centred treatment programmes, and planning safe discharges.

Working autonomously as a lone practitioner within the community, they will conduct holistic assessments that consider physical, psychological, social, and environmental factors contributing to falls risk, ensuring effective, patient-centred care and risk management.

Provide specialist falls prevention education and advice to patients, families, carers, and healthcare professionals.

Work collaboratively within multidisciplinary teams to deliver coordinated patient care.

Maintain accurate and timely clinical records in line with Trust policies.

Contribute to service development, clinical audit, quality improvement, and health promotion initiatives.

Ensure compliance with HCPC professional standards, CSP and clinical governance requirements.

Starting with NELFT NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

Probationary Period This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

High Cost Area Supplement – Outer London (B&D, Redbridge, Havering and Waltham Forest, Goodmayes) This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,870 to a maximum of £6,137p.a. pro rata for part time).
