The post holder will play a key role within Waltham Forest in supporting the delivery of evidence-based practice and monitoring patients with respiratory disease, particularly chronic obstructive pulmonary disease (COPD) in order to prevent further deterioration of their condition and to avoid unnecessary admission to hospital. The post holder will work closely with the Respiratory team to determine priorities within the allocated caseload in order to prevent crisis occurring and promote hospital avoidance. This will include carrying out home visits as directed by senior staff in order to maintain regular contacts with patients on the caseload to monitor their condition, assisting in the coordination of patients care across primary/secondary care and social services interface, and supporting patients in pulmonary rehabilitation. The post holder will have an active role in performing spirometry, smoking cessation, phlebotomy and clinical Audit.
Main duties of the job Modify intervention as needed depending on risk assessment, the situation and own skills and knowledge. Refer back to the registered nurse for support and advice ensuring that they are aware of any modifications made. Inform patients of facts using empathy, understanding and reassurance, adjusting communication style to the needs of the patient. This includes promoting and upholding the rights and diversity of individuals. Understand and deliver care in line with NELFT Safeguarding Policies (Adults and Children) Work collaboratively with other services. Order equipment & resources as agreed or directed by the registered nurse. Demonstrate, and instruct, on the use of equipment to ensure safety. Understand how to enable, move and position individuals in accordance with their plan of care. Report any incident/untoward incidents/near misses to self, patients or carers to the manager. Minimise the risk of spreading infection by cleaning, disinfecting and maintaining environments. Perform hand hygiene and use personal protective equipment to prevent the spread of infection. Recognise the need for further advice, guidance and support as appropriate. Undertake duties identified in each clients individual care plan which the post holder is trained, competent and confident to perform. Required to discuss issues of safeguarding with senior staff and ensure appropriate advice is sought and received when required.

An exciting opportunity has arisen for one healthcare assistants to join the Community Respiratory Team based at Jane Atkinson Health and Wellbeing Centre in Walthamstow London E17.
We are looking to recruit one part time (22.5Hres) healthcare assistant to work in this busy fast paced service and require keen and enthusiastic individual’s to join our dynamic multidisciplinary team.
You will work under the direct supervision of experienced nurses, in a supportive role, in both nurses led clinics and caring for people in their own homes, delivering quality care.
The successful applicant must be flexible, a good team worker, used to working under pressure, have excellent interpersonal skills and be able to communicate effectively with patients, carers and other members of the MDT.
The successful applicant will have a valued, challenging, and rewarding role within the team and will benefit from the support of a friendly hard working Integrated Community Team consisting of Nurses and therapists as well as other senior HCAs.

You will be expected to carry out clinical appointments within the Respiratory Service under the direction of the registered nurse. You will also be expected to carry out patient observation in clinical setting and in their homes. Perform spirometry testing in clinic. Assist patients with checking and using inhalers correctly. Assist with clerical and administrative tasks like booking appointments, tracking equipment.
The successful applicant must be flexible, a good team worker, used to working under pressure, have excellent interpersonal and IT skills and be able to communicate effectively with patients, carer, and other members of the MDT. The successful applicant would also need to hold an NVQ level 3 or equivalent qualification in health and social care .
We are interested in hearing from experienced healthcare assistants with proven clinical and communication skills to work as part of our friendly, supportive team. Previous experience in a community setting in the NHS is highly desirable but not essential.
The successful applicant will demonstrate the ability to utilise a range of skills and experience gained in clinical practice to assist the registered clinician to undertake comprehensive and holistic assessments.
Starting with NELFT NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.
