We are looking for a Salaried GP to join our friendly and hardworking team in Gorseinon, Swansea, working 24 sessions per week.

You will be joining a supportive practice and working alongside a dedicated multidisciplinary team, providing high-quality, patient-centred care to our local community.

We welcome GPs at all stages of their career, and a specialist interest would be particularly welcomed and supported. We offer a collaborative working environment where you will have the opportunity to develop your interests while being part of a committed and approachable team.

Main duties of the job

You will provide high-quality, patient-centred general medical services to our practice population.

Your main duties will include:

Provide high-quality, safe and effective clinical care, assessing, diagnosing and managing a broad range of acute and chronic conditions within general practice.

Conduct face-to-face and telephone consultations, taking a patient-centred approach and ensuring patients receive appropriate assessment, treatment, advice and follow-up.

Support patients with ongoing and complex health needs, taking a proactive approach to preventative care, health promotion and long-term condition management.

Work closely with our friendly and dedicated multidisciplinary team, including nursing, pharmacy and other healthcare professionals, to provide coordinated and comprehensive care for our patients.

Prescribe safely and appropriately, make referrals where necessary, review investigations and results, and ensure accurate and timely clinical documentation.

Contribute to clinical governance, significant event reviews, quality improvement and the ongoing development of practice services.

We welcome GPs with a specialist interest and are keen to support the successful candidate in developing their particular area of interest within the practice, where possible.

Work collaboratively with the GP partners and wider practice team, contributing to a positive, supportive and hardworking practice environment.

Job description Job responsibilities

The following are the core responsibilities of the Salaried GP.

The delivery of highly effective medical care to the entitled population

The provision of services commensurate with the primary care contract

To clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly referring patients to other services/agencies in a timely manner, utilising Practice and any other local guidelines

Generic prescribing adhering to local and national guidance

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care

Effective management of long-term conditions

To support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care

Processing of administration in a timely manner, including referrals, repeat prescription requests and other associated administrative tasks

Review medications following the appropriate policies, NICE/local clinical guidelines and local care pathways

Maintain accurate clinical records in conjunction with good practice, policy and guidance

Work collaboratively, accepting an equal share of the practice workload

Adhere to best practice recommended through clinical guidelines and the audit process

Support the delivery of anticipatory care plans

Contribute to the successful implementation of continuous improvement and quality initiatives within the practice

Accept delegated responsibility for a specific area (or areas) or the QOF

Attend and contribute effectively to practice meetings as required

Contribute effective to the development and maintenance of the practice including clinical governance and training

Be aware of your duties and responsibilities regarding current legislation and adhere to practice policies and procedures on Safeguarding Adults and Safeguarding Children

Ensure compliance with the appraisal process

Prepare and complete the revalidation process to remain fit to practice

Commit to self-learning and instil an ethos of continuing professional development across the practice team

Review and always adhere to practice protocols and policies

Encourage collaborative working, liaising with all staff regularly, always promoting a culture of continuous improvement

To be an integral part of the general practice team as well as supporting the wider network and multi-disciplinary team

There may be, on occasion, a requirement to carry out other tasks, this will be dependent upon factors such as workload and staffing levels

Wider responsibilities

In addition to the primary responsibilities, the Salaried GP has the following wider responsibilities:

Participate in the spectrum of clinical governance processes

Participate in formal training events promoting best practice in area of expertise Provide leadership and education for members of the multi-disciplinary team, providing guidance and support when necessary

Participate in local initiatives to enhance service delivery and patient care

Person Specification

Experience Essential

Ability to work both autonomously and as part of a multidisciplinary team Experience of working in a primary care environment Experience of providing home visits Minimum of two years as a salaried GP General understanding of the primary care contract Proven ability to evaluate the safety and effectiveness of own clinical practice Outstanding level of clinical knowledge and skills commensurate with that of an experienced GP Understanding of the importance of evidence-based practice Good MS Office and clinical system IT knowledge Understanding of the spectrum of clinical governance processes Excellent communication skills (written and oral) including recording of accurate clinical notes Ability to work within own scope of practice and understand when to refer to GP colleagues Experience of triage and the ability to effectively assess and diagnose Ability to listen whilst being honest and caring and be both sensitive and empathetic in distressing situations Understanding of the local pathology test processes and the ability to process the results and advise patients accordingly Ability to assess and manage acute deteriorations /exacerbations of long-term conditions Understanding of safeguarding adults and children Understanding of the importance of evidence-based practice
