About the role

We are looking for an enthusiastic, skilled and motivated Advanced Therapy Support Worker to join our newly established Early Supported Stroke Discharge (ESSD) team, delivering care across High Peak and North Dales.

This is a rare opportunity to be part of the development of a new service, working in line with the established and highly regarded ESSD model used across Derbyshire. You will play a key role in enabling people to leave hospital earlier and recover safely and effectively in their own homes, delivering high-quality, evidence-based stroke rehabilitation.

This post is part of exciting county-wide work to improve the stroke pathway across Derbyshire. You will be joining something truly meaningful at a pivotal moment, helping to shape how stroke rehabilitation is delivered to our communities.

Main duties of the job

To work with Qualified Therapists in the assessment and treatment of stroke patients as part of Early Supported Stroke Discharge Team.

To carry own caseload of patients and progress/modify intervention programmes within parameters set by the therapists.

To work alone in the patient's place of residence and/or clinic, under supervision but often without the presence of a qualified therapist.

To communicate effectively with patients and carers to maximise rehabilitation potential, as appropriate and under supervision.

To assess patient's needs, prescribe appropriate equipment and evaluate suitability and safety.

Whilst this role is primarily attached to the ESSD Service there may be times where flexibility across Stroke and Neuro Services is required due to operational pressures.

Qualifications Essential

NVQ Level 3 care/BTEC Health & Social care (or equivalent training/experience) Experience of working in a Healthcare setting

Experience Essential

Significant recent experience of working within adult/older people's rehabilitation Good knowledge and understanding of stroke and therapeutic approaches Previous experience of team working

Desirable

Experience of working in a community setting Experience of working as a Band 3 Therapy Assistant

Skills Essential

Excellent communication skills including verbal, written and IT Excellent organisational skills with ability to prioritise own workload Excellent manual handling skills Ability to work as part of a team or independently where appropriate Flexible and adaptable to patient/service needs Excellent interpersonal skills - experience of working with patients, families and carers
