About the role
We are looking for an enthusiastic, skilled and motivated Physiotherapist to join our newly established Early Supported Stroke Discharge (ESSD) team, delivering care across High Peak and North Dales.
This is a rare opportunity to be part of the development of a new service, working in line with the established and highly regarded ESSD model used across Derbyshire. You will play a key role in enabling people to leave hospital earlier and recover safely and effectively in their own homes, delivering high-quality, evidence-based stroke rehabilitation.

This post is part of exciting county-wide work to improve the stroke pathway across Derbyshire. You will be joining something truly meaningful at a pivotal moment, helping to shape how stroke rehabilitation is delivered to our communities.
Main duties of the job
To provide specialist Physiotherapy assessment and treatment for adults as part of the Early Supported Stroke Discharge team predominantly across High Peak and North Dales. There may be occasion where the post holder is asked to support the teams across Derbyshire where this is practicable, and capacity allows.
To work as a member of the multi-disciplinary team, working closely with therapists, support workers and nurses.
To educate and train other Physiotherapy staff, healthcare professionals, students, support workers and carers.
To participate in the development of effective, responsive, evidence based and patient centred services.
To lead/assist on specific projects in agreement with the Team Leader and Operational Lead for the Service.

Qualifications Essential
Degree in Physiotherapy HCPC Registration Post graduate experience and training in stroke rehabilitation
Skills Essential
Experience of completing specialist physiotherapy assessment and treatment of stroke patients Experience of working as part of a multidisciplinary team Evidence of understanding and application of clinical governance and evidence based practice Excellent communication skills Ability to work under pressure and manage own caseload
Knowledge Essential
Minimum 2 years post graduate experience Evidence of working in stroke rehabilitation
Desirable
Minimum 1 year working in a community setting
