St Margarets Medical Practice is seeking an experienced and highly motivated Advanced Nurse Practitioner (ANP) to join our friendly and forward-thinking primary care team.
We are a well-established GP practice serving approximately 13,000 patients in the heart of St Margarets, Twickenham. The successful candidate will play a key role in managing patient demand through clinical triage, ensuring patients receive timely access to the most appropriate healthcare professional and level of care.
The ANP will be responsible for reviewing and assessing online consultation requests submitted by patients, determining clinical urgency, and directing patients to the most appropriate clinician or service. This role is central to ensuring safe, effective, and patient-centred care while supporting efficient utilisation of the multidisciplinary team.
Main duties of the job
The successful post holder will:
Clinically assess patient requests submitted via the practice's online consultation system. Review patient-provided information and determine the appropriate clinical priority. Triage patients into urgent or routine pathways based on clinical need and risk assessment. Allocate patients to the most appropriate healthcare professional, including: General Practitioner (GP) Advanced Nurse Practitioner (ANP) Practice Nurse Healthcare Assistant (HCA) First Contact Physiotherapist (FCP) Clinical Pharmacist Physician Associate Community or external services where appropriate Manage same-day and urgent care demand safely and effectively. Provide autonomous clinical assessment, diagnosis, treatment, and prescribing (where qualified). Undertake face-to-face, telephone, and online consultations as required. Develop management plans and provide health promotion and preventative care advice. Escalate complex or high-risk cases appropriately. Contribute to safeguarding processes and identify vulnerable patients. Work collaboratively within the multidisciplinary team to ensure high-quality patient care. Support service development and continuous improvement initiatives within the practice. Maintain accurate and contemporaneous clinical records in accordance with professional and organisational standards.

Job SummaryThe Advanced Nurse Practitioner (ANP) will work as an autonomous practitioner within St Margarets Medical Practice, providing advanced clinical assessment, diagnosis, treatment, and management of patients presenting with a wide range of acute and chronic health conditions.
A key function of the role will be the clinical triage and management of patient requests received through the practice's online consultation system. The ANP will assess patient-submitted information, determine the clinical urgency of requests, and ensure patients are booked with the most appropriate healthcare professional or service based on their needs.
The post holder will play a pivotal role in managing patient demand, supporting timely access to care, improving patient outcomes, and ensuring efficient utilisation of the multidisciplinary team (MDT).

Qualifications Essential
Registered Nurse with current registration with the Nursing and Midwifery Council (NMC). Master's level qualification in Advanced Clinical Practice or equivalent experience. Independent Non-Medical Prescriber qualification. Evidence of continuing professional development.

Advanced Clinical Practice MSc. Minor illness and/or minor injury qualification. Teaching or mentorship qualification.
