Job summary
This role will be directly employed by St Margarets Medical Practice working as part of Bentworth PCN. We are seeking an experienced and motivated Clinical Pharmacist to join our friendly and forward-thinking multidisciplinary team.
Contract: Full Time
Salary: Band 7 equivalent (dependent on experience)
The post holder will work as part of the practice multidisciplinary team to provide medicines optimisation, structured medication reviews and prescribing support to patients and clinicians. The Clinical Pharmacist will contribute to safe and effective medicines use, support long-term condition management, and assist with medicines governance activities across the practice.
The role offers an opportunity to work closely with GPs, nurses, pharmacy technicians and the wider primary care team to improve patient outcomes through high-quality pharmaceutical care.
Main duties of the job
The Clinical Pharmacist will provide specialist clinical pharmacy services within the GP practice, supporting medicines optimisation, prescribing safety and patient care.
The post holder will work collaboratively with the GP Partners, pharmacy technician team and wider multidisciplinary team to ensure the safe, effective and evidence-based use of medicines.
Key responsibilities include:
Delivering Structured Medication Reviews (SMRs), particularly for patients with complex needs and polypharmacy. Supporting medicines optimisation and deprescribing initiatives under supervision of the GP. Managing medicines-related queries from patients and clinicians. Supporting long-term condition management within areas of competence. Undertaking medicines reconciliation following hospital discharge. Contributing to prescribing safety audits and quality improvement initiatives. Supporting implementation of local and national prescribing guidance.

Our multidisciplinary team consists of 3 GP Partners, 7 Salaried GPs, a Physician Associate, a Practice Nurse, 3 Healthcare Assistants, 3 GP Assistants, and an experienced reception and administrative team. Clinical Practice and Medicines Optimisation
Undertake patient-facing Structured Medication Reviews (SMRs). Optimise medicines use for patients with long-term conditions and complex medication regimens. Provide medication reviews following hospital discharge and care transitions. Support repeat prescribing processes in line with practice policies. Respond to medication-related queries from patients and healthcare professionals. Promote shared decision-making and medicines adherence. Support management of long-term conditions including diabetes, asthma, COPD and hypertension within scope of practice and competence and under the supervision of GPs. Medicines Safety and Governance
Support the implementation of medicines optimisation and prescribing safety initiatives within the practice. Contribute to medicines-related audits, quality improvement projects and service evaluations. Assist in monitoring prescribing performance against local and national guidance. Support the review and implementation of MHRA alerts, NICE guidance and safety communications. Participate in significant event reviews and learning activities relating to medicines management. Contribute to the maintenance of safe prescribing systems and processes. Escalate medicines-related risks and concerns appropriately. Multidisciplinary Team Working
Work collaboratively with GPs, nurses, pharmacy technicians and administrative teams to improve patient care. Provide medicines advice to members of the multidisciplinary team. Liaise with community pharmacies, secondary care providers and ICB medicines optimisation teams where required. Support implementation of local and national medicines optimisation initiatives. Contribute to achieving practice and PCN quality improvement objectives. Clinical Governance
Maintain accurate and legally compliant clinical records. Work in accordance with GPhC standards, NHS policies and practice procedures. Person Specification
Qualifications Essential
Completion of an MPharm Registered Pharmacist with the General Pharmaceutical Council (GPhC). Right to work with in the UK
Desirable
Clinical Diploma or equivalent postgraduate qualification. Independent Prescribing qualification with experience of prescribing in primary care or working towards one. Completion of the Primary Care Pharmacy Education Pathway (PCPEP) or equivalent experience.
Experience Essential
Minimum of 2 years post-registration clinical pharmacy experience. Experience in a patient facing clinical pharmacy role. Experience undertaking structured medication reviews. Experience of medicines optimisation. Experience of working within multidisciplinary healthcare teams. Experience of managing patients with long-term conditions.
Desirable
Experience working within General Practice or Primary Care Networks. Experience of quality improvement, audit and service development. Experience as an Independent Prescriber in primary care.
Knowledge and Skills Essential
Strong clinical knowledge and application of evidence-based prescribing. Ability to undertake comprehensive medication reviews. Understanding of medicines optimisation and prescribing safety principles. A understanding and ability to interpret relevant blood test results to optimise medication and patients chronic health needs as appropriate. Excellent communication and consultation skills. Ability to work within professional competence.
Desirable
Knowledge of SystmOne. Understanding of QOF, IIF and PCN service requirements. Familiarity with local medicines optimisation programmes and formularies.
