St Margarets Practice is seeking an experienced and motivated Prescribing Clinical Pharmacist to join our friendly, forward-thinking multidisciplinary team.
This is a patient-facing role within a GP Practice, with additional responsibility for acting as the practice Medicines Management Lead and leading the pharmacy team which consist of a pharmacy technician and prescription clerk team. The role offers the opportunity to deliver high-quality Medicines Optimisation, provide clinical leadership, and shape prescribing safety and governance across the practice and work as a key member of the wider MDT and practice team to improve patient care.
Main duties of the job
The Senior Pharmacist is a senior clinical role within the GP practice, providing leadership and assurance for medicines optimisation, prescribing safety and medicines governance. The post holder will act as the practice lead for medicines management, working closely with partners, and the pharmacy team, and the Integrated Care Board (ICB) to ensure safe, effective and legally compliant use and supply of medicines. They will also run clinics to support effective medicines optimisation through medication reviews, deal with prescription queries and help with the management of long term conditions including Diabetes, Asthma, COPD and hypertension.

Medicines Management & Prescribing Safety
Act as the practice Medicines Management Lead, providing clinical assurance to Partners Lead the development, implementation and review of medicines management policies and SOPs Maintain oversight of prescribing safety, repeat prescribing and medicines-related risk Own and update medicines-related risks, help to mitigate risk and feedback to the clinical team as part of the PCN. Lead medicines-related significant event analyses and learning Act as the practice link for ICB medicines optimisation teams and achieve relevant MOES targets. Ensure practice prescribing is aligned to ICB formulary and local policies Lead or contribute to ICB-requested audits, improvement work and reporting Represent the practice at medicines-related system meetings where requiredPrescribing & Clinical Practice
Undertake complex structured medication reviews and polypharmacy optimisation Answer medication queries from patients Undertake clinics to support with chronic condition management with in competency Undertake post hospital discharge reviews of patients and support medicines optimisation. Providing clinical oversight of SOPs and governance in relation to medication safety. Ensuring prescriptions are clinically authorised prior to dispensing. Supporting safe handling of controlled drugs, high-risk medicines and emergency supply. Advising on clinical implications of stock shortages and therapeutic alternatives. Work in line with ICB medicines optimisation policies, formularies and local prescribing guidance. Work with the other senior and junior pharmacists with in the PCN to help achieve PCN targets and objectives. Support implementation of national and local prescribing initiatives and safety campaigns. Liaise with ICB medicines optimisation teams, community pharmacy and secondary care as required. Provide clinical input to ICB requested audits, data returns or improvement work.Clinical Governance & Safety
Comply with GPhC standards, GP Contract requirements and practice policies, working within professional scope and competence. Maintain clear, accurate and contemporaneous clinical records. Contribute to significant event analysis, prescribing incidents and risk register updates. Lead and act on MHRA alerts, safety notices and NICE guidance relevant to general practice and dispensing. Escalate concerns appropriately to the Partners. Conduct and produce clinical audits and governance policies in response to nationwide reporting and incident and pattern identification. Education
Keep up to date with professional and clinical issues that affect the work of this post. Attend statutory training and carry out CPD requirements as specified by the General Pharmaceutical Council. Ensure that mandatory health and safety and other training required by the GP Practice is kept up to date. Undertake any additional training that is considered relevant to the role. Participate in training and supervision of foundation year pharmacists if needed, medicines management technicians and induction of staff as requested as part of the Medicines Management Team.
Person Specification
Experience Essential
Qualified and working in a clinical setting for over 4 years.
Desirable
Experience working GP practice. Experience in medicines management leadership, audit or service development. Experience in Clinical pharmacy/ hospital setting Experience managing a conducting own clinics.
Qualifications Essential
GPhC-registered Pharmacist. Independent Prescriber qualification. Completion of the Primary Care Pharmacy Education Pathway
Desirable
Completion of clinical pharmacy diploma or equivalent
