About this role

Job summary

The post holder will provide clinical leadership for the Fracture Liaison Service (FLS) within Hywel Dda University Health Board, acting as a highly specialist practitioner with expert knowledge in bone health. Working autonomously within agreed protocols, they will manage a complex caseload while supporting the delivery of the service plan alongside the Clinical Lead for Bone Health.

The role requires a high degree of professional autonomy, enabling the post holder to make critical clinical judgements and deliver effective, evidence-based care. They will play a key role in service modernisation, contributing to the planning, development, and evaluation of the service, while maintaining a strong clinical profile and championing high standards of patient-centred care.

Vacancies are available in the Ceredigion / Pembrokeshire /Carmarthenshire areas. Location to be discussed upon appointment. Both full time and part time hours will be considered.

Main duties of the job

The post holder will deliver a high-quality, specialist clinical service by undertaking advanced assessment, diagnosis, and management of patients with osteoporosis and fragility fractures, including running FLS clinics, interpreting investigations such as DXA scans, and developing personalised care plans.

They will strive towards the constant improvement of KPI's in line with national standards as well as the meeting of targets, as indicated. They will communicate complex and sensitive information effectively, provide expert clinical advice, and ensure accurate reporting and discharge planning. In addition, they will lead on clinical governance activities including audit at both a local and national level, research, and service evaluation, and contribute to the development and maintenance of clinical standards across the Health Board.

The role includes supervising and training staff, developing competency frameworks, and delivering education to multidisciplinary teams and external partners. The post holder will promote patient autonomy, support self-care, identify and minimise risks, and actively contribute to continuous service improvement while maintaining a balance between independent practice and collaborative team working.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Person Specification

Qualifications and Knowledge Essential

Educated to Degree level or equivalent experience Evidence of leadership courses/ qualifications. Master's Degree or equivalent level of specialist experience in area of clinical expertise. Professional registration (e.g NMC, HCPC, GPhC, PAMVR)

Desirable

Non-medical prescriber.

Experience Essential

Experience of working within Medicine, Orthopaedics or related speciality. Evidence of advanced clinical practice in area of speciality. Evidence of MDT working Experience in consultant led clinics Significant experience working as an autonomous practitioner Experience managing a complex caseload using an advanced clinical reasoning process Experience in delivering audit Experience in delivering training to staff on complex matters

Desirable

Experience of referring for and analysing the results of tests Experience in assisting in leading service developments
