Job summary Band 5 A&C Discharge Flow Coordinator (St Helier site )

We are seeking an energetic, dynamic, and enthusiastic individual with strong organisational skills for the role of DischargeFlowCoordinator.

The purpose of the DischargeFlowCoordinator role is to ensure that the patient's journey from hospital admission through to the discharge planning to return home or to another care facility are smooth and safe. The main purpose is to optimise hospital bed availability , reduce lengths of stay and prevent delay discharge by working closely with MDT's

Main duties of the job

We are seeking an energetic, dynamic, and enthusiastic individual with strong organisational skills for the role of DischargeFlowCoordinator.

The purpose of the DischargeFlowCoordinatorrole is to ensure thatpatients have a seamless journey on discharge and that the patient's pathway is effectively managed across site. The primary duty is to facilitate safe and timely discharges, ensuring patient do not stay longer than necessary in acute hospital bed,working closely with doctors, nurses,social workers and therapist to coordinate care packages , arranging patient transport , home equipment and referrals to external care providers, acting as the primary point of contact for families and escalating any delayed on transfer of care
