Job summary

An exciting opportunity has arisen to appoint a Band 6 health care professional to join the Rapid Access Acute Rehabilitation service in a Case Manager Role.

This post will be based in the RAAR (Rapid Access Acute Rehabilitation) service, a specialist service providing level 1 inpatient neuro-rehabilitation at an 8 bedded unit at St George's Hospital.

The Case Manager will maintain adequate flow through the 8 bedded RAAR service by proactively liaising with external providers and agencies including but not limited to specialist rehabilitation providers, bed managers, community therapy teams, social services, housing services, equipment providers, and third sector providers.

The post holder will work autonomously to support and co-ordinate, where necessary, the case management of individuals with highly complex health and social care needs. The post holder will be working to support, individuals and their families/carers in accessing the most appropriate care to meet their needs.

Main duties of the job

The post holder will work autonomously to support and co-ordinate, where necessary, the case management of individuals with highly complex health and social care needs. The post holder will be working to support, individuals and their families/carers in accessing the most appropriate care to meet their needs.

The post holder will be required to have a good knowledge of health and social care practice and be able to demonstrate an understanding of the interaction between hospital discharge and Continuing Healthcare and Funded Nursing Care, and be prepared to extend their skills, knowledge and competence in this field.

Job description Job responsibilities

Ensure that case management is delivered with the patient at the heart of the process and that case management issues are dealt with in line with best practice in a timely manner to achieve best outcomes for the individual.

Make decisions regarding complex case management issues in line with relevant guidance and with support of senior colleagues where needed.

Oversee and advise on the safe and appropriate transition of complex patients to their next placement on a rehabilitation trajectory to ensure flow through RAAR.

The post holder will be required to regularly review individuals and may be required to assist with collating comprehensive reports providing assurances to relevant stakeholders such as the ICB, other rehabilitation providers, social services, housing partners that plans are appropriate.

Initiates and participates in evidence-based projects and is responsible for implementing changes within designated team/area and setting and monitoring standards of practice.

Facilitate with processes such as D2A, onward referrals, equipment provision, in order to ensure timely discharge from the RAAR service.

Diploma/Degree in Physiotherapy/ Occupational Therapy/ Nursing/ Speech and Language Therapy
