About this role

Job summary

As a member of the MS multi-disciplinary team, the post holder will work as part of a collaborative working project to improve patient outcomes and quality of care. The post holder will support the Multiple Sclerosis nursing team in coordinating and managing clinic bookings to identify patients that meet the project criteria. The post holder will be responsible for the collection and monitoring of data pertaining to the service and contributing to service development.

Main duties of the job

Facilitate and co-ordinate the care pathway of patients:

1. Ensure relevant investigations (e.g. MRI, CSF results) are available to Consultant Neurologists and Clinical Nurse Specialists to enable comprehensive assessment and treatment.

2. Support MS Clinical Nurse Specialist in booking and arranging appointments for patients allocated to the project

3. Entering project data according to instructions to achieve project's goals

4. Taking d triaging telephone and email messages from patients and other healthand social care professionals and directing them to the appropriate clinician or admin team.

5. Be responsible for analysing and resolving information queries to the MS team coordinator promptly.

6. Establish and ensure the continuation of effective communication between the MDT team, external healthcare delivery companies, injection support nurses, regional neurologists, regional MS nurses, GPs and the patient to enable seamless provision of DMTs.

7. Monitoring patients' attendance, following up as appropriate, particularly treatment-nave patients, patients with cognitive difficulties and those taking DMT.

8. Telephoning patients with identified cognitive difficulties to remind them of appointments to reduce non-attendance.

Qualifications Essential

Educated to GSCE level or equivalent Excellent computer skills including being competent with Microsoft Word, Excel, and Powerpoint

BSc or equivalent

Skills Essential

Excellent communication skills Evidence of good organisational skills

Experience in data entry and audits

Experience Essential

Experience of working in a customer facing environment Experience of using databases and IT programmes Experience in working in the Multiple Sclerosis Field for 1 year
