About this role

Job summary

To ensure the provision of expert knowledge and research based practice to all adults with Diabetes across the catchment area covered by South Tees NHS Foundation Trust. To perform dynamic endocrine testing within the Diabetes Directorate.

To provide senior clinical support and on-going education for all staff caring for people with Diabetes.

To provide a specialist diabetes service delivering and ensuring the highest standard of nursing care at all times. The post holder will give advice on all nursing matters relating to diabetes, acting as an expert resource for the MDT and all other agencies involved in the care of people with Diabetes.

The post holder will lead the assessment, planning, implementation and evaluation of patient care in consultation with the MDT and other organisations to address individual needs involving patients and their carers in disease management, whilst encouraging patient participation in care and service development.

To be the involved in the initiation and facilitation of research projects, clinical audit, national audit and national peer review of the service in order to demonstrate clinically effective practice.

To establish, support and continue to maintain nurse and dietetic led clinics to support patients with diabetes.

To maintain continuous professional development and training in diabetes care.

Main duties of the job

Key Results and deliverables:

Patient Support

To provide an on-going support service for both patients and carers

Plan provision for continuity of care, referring and admitting patients as necessary

To prepare appropriate written information consistent with best practice and Trust guidelines to enhance understanding

To communicate diagnosis and treatment options for people with diabetes

Provide inpatient visits and outpatient clinics

To provide telephone access for patients and carers to allow on-going support and to facilitate access to members of the diabetes team for information and advice

Work with colleagues to develop patient information within the specialty, acting as an advocate on behalf of both service users and carers when necessary

Promote awareness regarding health promotion and prevention; this may on occasions include partnership working with Primary Care Colleagues.

Nurse Led Clinics

To support and continue to maintain nurse led element of MDT clinics to support patients with diabetes

Provide a nurse led follow up service from diagnosis

Discuss with the patient the outcome of consultation, initiate appropriate investigations and treatments necessary and monitor possible outcome, including breaking bad news when necessary

Make autonomous clinical decisions.

Person Specification

Experience Essential

Ability to work autonomously or as a member of a team and multi-disciplinary team Ability to manage stressful situations, challenging decisions. Ability to motivate self and others.

Desirable

Experience in providing counselling to patients.

Qualifications & Training Essential

Registered Nurse BSC ENB 998 or equivalent

Desirable

Non-medical prescribing

Knowledge & Skills Essential

Significant post registration experience at band 5 or above. Advanced teaching skills. Advanced communication and interpersonal skills. Ability to work under pressure. Ability to organise, prioritise work of self and others. Computer Literate.
