Job summary
Your Life Your Way are looking for a compassionate and motivated Care Coordinator to join our Team, supporting adults & children with both social and complex health needs.
Main duties of the job
This role will be based at our Head office in Hartford, Cheshire 3 days per week and home working 2 days per week however you may also be required to commute to client homes across the Northwest at times of service need.
