About this role

Job summary

We are seeking a motivated and compassionate Care Coordinator to join our multidisciplinary Primary Care Network (PCN) team on a part-time basis.

This is a key role supporting patients, particularly those with long-term conditions, frailty, dementia, or complex needs, to navigate health and social care services. You will act as a central point of contact, working closely with GP practices and partner organisations to ensure patients receive coordinated, personalised care.

The role combines administrative coordination and patient-facing work, including engaging directly with patients and carers to support their health and wellbeing.

Main duties of the job

Proactively identify and support patients who would benefit from care coordination, including vulnerable or high-risk groups. Act as a central point of contact for patients, carers, and professionals to help navigate services and access appropriate support. Develop, implement, and review personalised care and support plans based on what matters most to the individual. Coordinate care across a range of services, including primary care, community services, and voluntary organisations. Work closely with GPs and the wider multidisciplinary team (MDT), including attending and supporting MDT meetings. Support patients to improve self-management, access support services, and make informed decisions about their care. Maintain accurate and up-to-date patient records, ensuring compliance with information governance requirements. Provide signposting to internal and external services, including social prescribing and community resources. Undertake follow-up activity to ensure continuity of care following hospital discharges or clinical interventions.

Person Specification

Qualifications Essential

Experience working in healthcare, social care, or a community-based role. Excellent communication and interpersonal skills, with an empathetic and patient-centred approach. Strong organisational and administrative abilities, with good IT skills. Ability to manage a varied workload and prioritise effectively. Commitment to delivering high-quality personalised care. GCSE (or equivalent) in English and Maths

Desirable

Experience working with older people, dementia patients, or those with complex needs. Knowledge of primary care systems (e.g. EMIS, SystmOne). Understanding of the NHS, social care, and voluntary sector services
