Job summary

We are looking to recruit an enthusiastic and highly motivated Patient Pathway Coordinator to the Dermatology Team at St. George's Hospital. on a fixed term contract in the first instance.The pathway coordinator post involves liaising with clinicians to agree dates for MOP's and appointments, contacting, and booking patients. It will also involve cancelling and rescheduling patient admissions and clinics where necessary. The post holder will manage waiting lists; undertake validation of the data, department PTL and escalating any issues to the assistant service manager. The post holder must be comfortable engaging with patients as well as clinical teams.The successful candidate will need to be organised and confident and be able to multi-task and prioritise work. Previous experience as a patient pathway co-ordinator ormedical secretarywill be beneficial as will a working knowledge of the Cerner Millennium system.

Main duties of the job

Key tasks of the role are:

To ensure the smooth running of Dermatology appointments. To schedule appointments in liaison with patients, consultants and capacity. To manage/ validate on a weekly basis the PTL's for Dermatology and escalate any possible breaches or capacity issues To ensure accurate and complete computer records are maintained for patients. To contribute to service developments. To ensure all appointments/ procedures are coded correctly and coding is clear within the cerner system. Prep clinic notes at short notice for Dermatology or Lymphedema appointments

The post holder will be responsible for ensuring that patients are booked on the relevant MOP's (minor operation) list prioritising clinical urgency and breach dates, as well as insuring that MOP list are fully utilised. The post holder will also be responsible for supporting the coordination of patient's pathways through appointment booking and out coming of clinics.

The post holder will also be expected to cross cover other admin members in the team during periods of leave.

Responsible for ensuring compliance with Trust policies and local protocols for Data Protection, Health and Safety, Security and Fire Safety within the administration team.

Person Specification

Experience Essential

Proficient user of Microsoft Office package (e.g. Word, Excel, PowerPoint, Access) Able to produce reports and process information Experience in validating and managing data and information

Desirable

Previous Healthcare experience Proficient using PAS or equivalent hospital administration system.

Qualifications Essential

3 GCSE's or equivalent including English & Maths Grade C and over NVQ 3 in Office Administration office administration experience

Skills Essential

Deals constructively and sensitively with complaints Able to prioritise own work in an environment of changing and sometimes conflicting priorities Builds and maintains sound relationships within teams and with colleagues in other areas including corporate departments.

Knowledge Essential

Awareness and understanding of national policy concerning patient care and developments within the NHS

Desirable

Knowledge of medical terminology An understanding of 18 weeks, diagnostic, cancer pathways and trust access policy
