Job summary We have an opportunity for a band 6 Discharge Facilitator Specialist Nurse within Trauma and Related Services Department (TRS).
Main duties of the job You will be implementing, coordinating and facilitating safe and timely discharges for patients within the TRS bed base. You will follow their pathway throughout their journey through all of our services from the initial admission through to discharge. You will be the main contact for families and carers supporting them during the discharge process.

Job description Job responsibilities Why you should join our Team
You will be responsible for leading a team of band 3 discharge coordinators and act as senior support for TRS in the absence of the band 7 discharge lead. The role requires attendance at MDT daily where you will alongside the multidisciplinary team identify same day and future discharges, any delays in service that will prevent a smooth transition from hospital to home. The role requires you to remain in close contact with the operational matron by attending twice daily patient flow meetings to provide discharge numbers and escalations to support early discharges.
What you will bring to the role The discharge team operate a seven-day service and you will be expected to work some weekends and bank holidays.
We will provide local training for the successful candidate and you must be a highly developed communicator as this is essential to the role.
