About this role

Job summary

We have an exciting opportunity for a registered adult nurse to join our friendly and dynamic clinical team at Somer Valley Medical Group for 30 hours per week.

The role will be delivering all general practice treatment room duties to our patient population with opportunities for career development.

We are looking for a motivated, innovative, and ambitious nurse with excellent communication and interpersonal skills. Ideally you will have some previous primary care experience, however it would also suit a nurse who wants to pursue a career within general practice.

Main duties of the job

The post holder is responsible for ensuring the delivery of safe and effective patient care to the practice population using autonomous clinical skills and a broad in depth theoretical knowledge base.

The post holder will manage a diverse clinical case load and provide clinical nursing services within treatment room clinics, including wound management, adult and children's vaccinations, and national screening programs within the professional competence of the post holder.

The post holder will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures and all duties will be carried out in accordance with the NMC Code of Professional Conduct.

Somer Valley Medical Group includes 3 sites in the Midsomer Norton area. You will be working within our excellent team of 15, which include Nurses, GPAs, HCAs and Phlebotomists.

Job description Job responsibilities

Meeting patient's nursing care needs as identified, or where care has been transferred to the nurse by a GP or other senior clinician. This will usually be in the surgery, but occasional home visits may also be required.

Managing the nursing care of patients, employing personal recall systems suitable to the practice needs. This includes using the practice computer systems for input of information for recall and screening purposes. Accurate and efficient input of data for contract (QOF, LCS) purposes is also required.

Deliver nurse led care including:

Cervical cytology samplingChildhood immunisationsTravel vaccinationsSeasonal vaccinationsLeg Ulcer management including Doppler assessments and compression therapyWound care and assessing and documenting wound care plans.Prioritise health problems and recognising a deteriorating patient and appropriately refer to the Duty GP.Provide training and supervision for healthcare assistants within Treatment Room clinics.Participate in the education and training of students of all disciplines and in the induction of all members of practice staff.Adhere to the Nursing and Midwifery Council (NMC) Professional Standards of Conduct and Behaviour (The Code).Responsible for the management of treatment room provision, ensuring stock levels have been ordered and maintained by junior staff.Administration of immunisations and medications under PGDs, PSDs and National Protocols.Administer medications as prescribed and in accordance with NMC Standards, e.g. via nebuliser or injections using sub-cutaneous or intramuscular route under PGDs, PSDs and National Protocols.Skin suture and clip removalVenepunctureECGsVital signs measurementsProviding health education advice around diet/smoking/lifestyle factors Management of patients with long term conditions. (e.g. diabetes)

- BSc Hons in Adult Nursing or Diploma of Higher Education (DipHE) in Nursing - Current NMC Registration (Level 1) Adult Nurse

Desirable

-Post-Graduation study or training courses in topics allied to General Practice or Community nursing. -Teaching/Mentor Qualification

Experience Essential

-A Minimum of 2 years post registration experience -Administration of medications under PSDs/PGDs/VGDs -Computer skills -Experience of using an electronic Patient record System either TPP System One, EMIS Web or similar. -Excellent communication skills, both written and verbally -Time management skills and able to work autonomously managing own workload.

Desirable

-Primary Care experience. (e.g. district nurse or a General Practice nurse) -Doppler assessments -Compression therapy -Venepuncture -Wound Care -Cervical Cytology (Sample Taker) -Travel Health -Vaccinations (adults/childrens) -Participation in audits, quality improvement initiatives or projects -Experience of training and supporting non-registered staff (nursing assistants or nursing associates)
