Job summary
We are seeking experienced registered nurses to join our Childrens Continuing Care service as AACC Senior Nurse Assessors for Children and Transitions. This role offers the opportunity to improve outcomes for children and young people with complex health needs, keeping their voice and the views of their families central to decision-making. You will undertake complex assessments and reviews, coordinate multi-agency input, manage a caseload and support young people to move safely and smoothly into adult services. We are looking for skilled, organised and compassionate clinicians with experience working with children and who can work autonomously, chair complex discussions and build strong relationships across agencies.
This role will require weekly office-based working in line with the hybrid working policies set out for each ICB, with office bases available in Coventry, Warwick, Worcester and Hereford for all staff to use. While this role is expected to be place-based, travel will be required across the cluster and to care settings as needed.

Main duties of the job
You will undertake complex, holistic assessments and reviews in line with the National Framework for Children and Young Peoples Continuing Care, and prepare and present assessments to the Childrens Continuing Care Panel. You will manage a defined caseload and support the commissioning, monitoring and review of individualised packages of care.
Working with children, young people, families and partners across health, social care, education and the voluntary sector, you will coordinate a strong Team Around the Child approach and ensure care is integrated and outcome-focused. You will contribute to planning timely transitions into adult services through joint assessment and coordinated care. A key part of the role will be contributing to safeguarding, governance, audit, learning and service improvement, as well as providing peer support or deputising for the Clinical Team Manager when required.

Assessment and eligibility
Undertake complex, holistic assessments and reviews for children and young people with families, carers and multi-agency professionals.
Apply the National Framework for Children and Young Peoples Continuing Care consistently to inform evidence-based eligibility decisions.
Prepare and present comprehensive assessments to the Childrens Continuing Care Panel.
Case management and care coordination
Manage a defined caseload and coordinate the timely review and monitoring of care packages.
Work within commissioning processes to secure safe, effective and value-for-money packages that meet assessed needs.
Support accurate and timely health input to Education, Health and Care Plans.
Transitions and partnership working
Facilitate safe and timely transitions for young people moving into adult services.
Work with adult CHC teams and partner agencies to support joint assessments and continuity of care.
Coordinate multi-agency working across health, social care, education and the voluntary sector through a Team Around the Child approach.
Communication, quality and leadership
Ensure the voice of the child or young person and their family or carers is central to care planning and decision-making.
Contribute to safeguarding, clinical governance, audit, evidence-based practice and service improvement.
Provide peer support and supervision where appropriate and deputise for the Clinical Team Leader when required.
Maintain current NMC registration and practise in line with the NMC Code and organisational policies.
Person Specification
Experience Essential
Recent experience in complex care, particularly with children and young people Extensive experience of working with children and young people within a relevant specialty Experience of assessment and review processes using recognised assessment tools Experience of multidisciplinary working

Experience within Continuing Care Experience of delivering or supporting training
Additional skills, knowledge and qualities Essential
Awareness and understanding of the Childrens Continuing Care Framework and its application Excellent nursing assessment and clinical judgement Strong written, verbal, interpersonal, organisational and time-management skills Ability to work autonomously, chair multi-agency meetings and prioritise competing demands Full driving licence and ability to travel as required
Qualifications Essential
Current NMC registration as RN Child/RSCN/SCPHN, or as a Learning Disability or Mental Health Nurse with extensive caseload experience Educated to degree level or equivalent experience in healthcare services Safeguarding Children Level 3 Evidence of continuing professional development

Postgraduate teaching certificate or module
