Job summary
We are seeking experienced registered nurses to join our All Age Continuing Care team as AACC Senior Nurse Assessors. This is an opportunity to use your clinical judgement, assessment skills and partnership-working experience to support fair, robust and person-centred decisions for people with complex health needs. You will undertake assessments and reviews, coordinate multidisciplinary input and manage a defined caseload across a range of care settings. We are looking for confident and compassionate clinicians who can analyse complex information, communicate sensitively and maintain high professional standards in a changing service.
This role will require weekly office-based working in line with the hybrid working policies set out for each ICB, with office bases available in Coventry, Warwick, Worcester and Hereford for all staff to use. While this role is expected to be place-based, travel may be required across the cluster and to care settings as needed.
NHS Coventry & Warwickshire ICB and NHS Herefordshire & Worcestershire ICB are working together as a cluster with a shared management team. Main duties of the job
You will undertake comprehensive CHC and FNC assessments and reviews, including Fast Track pathways, using the relevant National Framework to support evidence-based recommendations. You will manage a caseload, monitor and review care packages, assess risk and contribute to safe, responsive care planning.
You will coordinate multidisciplinary activity and work closely with health, social care, independent and voluntary sector partners. You will explain complex and sensitive decisions to individuals, families and carers, identify and escalate safeguarding or quality concerns, and contribute to complaints, quality assurance, clinical governance and service improvement. You will also support transition planning for young people moving from Childrens Continuing Care into adult CHC services.

Assessment and eligibility
Undertake comprehensive assessments of health and care needs across acute, community and care home settings.
Apply the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care, including Fast Track pathways, to support eligibility decisions.
Ensure assessments are holistic, evidence-based, person-centred and completed within agreed timescales.
Clinical decision-making and case management
Analyse complex clinical information, assess risk and make informed recommendations regarding eligibility and care needs.
Manage a defined caseload and review, monitor and evaluate care packages to ensure they remain safe, effective and responsive to changing needs.
Contribute the NHS perspective to joint care planning and jointly funded packages with social care partners.
Communication and partnership working
Coordinate and contribute to multidisciplinary assessments and reviews and build effective relationships across health, social care, independent and voluntary sectors.
Communicate complex, sensitive and potentially distressing information to individuals, families and carers with empathy and clarity.
Support transition planning for young people moving from Childrens Continuing Care into adult CHC through early identification, joint planning and timely decision-making.
Quality, governance and professional practice
Identify and escalate safeguarding concerns and quality issues in line with policy.
Contribute to complaints, patient experience, quality assurance, clinical governance and service improvement.
Maintain current NMC registration, practise in line with the NMC Code and undertake continuing professional development.
Person Specification
Qualifications Essential
First-level nursing qualification (RGN, RMN or RNLD) with current registration Relevant degree or equivalent knowledge, skills and experience Evidence of continuing professional development
Experience Essential
Significant post-registration experience Experience of working with individuals with multiple and complex health needs Experience of effective multidisciplinary and cross-organisational working Ability to undertake nursing needs assessments, analyse information and make recommendations regarding CHC/FNC eligibility

Experience or ability to chair multidisciplinary meetings
Additional skills, knowledge and qualities Essential
