We are seeking an enthusiastic Practice Nurse to join our dedicated, friendly team at Talybont Surgery. The Practice Nurse will provide high-quality nursing care to patients in a primary care setting.

Main duties of the job

Administer injections, including childhood immunisations, travel vaccines and many others

Provide wound care, managing dressings and referring into secondary care where appropriate

Provide smear checks

Provide advice and monitoring for chronic disease management

Communicate with other health professionals to ensure comprehensive patient care

Maintain compliance with practice protocols, guidelines, and national clinical standards.

Talybont Surgery is a friendly, progressive, partner-led practice committed to delivering high-quality patient care within a supportive multidisciplinary environment.

Our clinical team currently consists of six GPs (including four salaried and two partners), two GP Registrars, two Prescribing Pharmacists, a Mental Health Practitioner, a Womens Health Specialist Nurse, two Practice Nurses and a Healthcare Assistant (HCA). The clinical team is supported by an experienced and friendly administrative, reception and management team.

We are also a training practice and are actively involved in supporting the development of future clinicians. We currently host two GP Registrars as well as undergraduate medical students, and the successful candidate will be expected to contribute to a supportive teaching and learning environment within the practice.

Job description Job responsibilities

The Practice Nurse will provide high quality nursing care as part of our dedicated nursing team, working with the wider practice to deliver outstanding patient care. The main focus will be on general nursing duties such as dressings and immunisations as well as ensuring there is efficient stock for upcoming appointments. The Practice nurse will also play a crucial role in the delivery of our flu and Covid vaccines.

Key Responsibilities

Clinical Duties

Chronic Disease Management: Provide reviews, and ongoing management of patients with conditions such as diabetes, asthma, COPD, or hypertension. This will depend on experience.

Childhood Immunisations: Administer vaccinations following national guidelines and maintain accurate records. This will involve working closely with the Health Visitors.

Wound Care & Dressings: Provide treatment and follow-up care for wounds, ulcers, burns and post-operative dressings. Refer into secondary care for more complex wounds.

Health Promotion & Prevention: Offer smoking cessation support, weight management, and lifestyle advice to patients during consultations where appropriate.

Womens Health: Conduct cervical screening and womens health advice where appropriate.

Treatment Room Duties: Assist with maintaining a clean, tidy environment with adequate stock levels.

Team Collaboration & Professional Development

Work closely with the wider team including GPs, Pharmacists and non clinical teams.

Engage in team meetings to discuss complex cases and contribute to practice development. This may involve practice, clinical or palliative care meetings.

Participate in continuous professional development (CPD) and maintain up-to-date clinical skills and knowledge.

Assist in audits, quality improvement projects, compliance and policy updates within the practice.

Administrative & Compliance Responsibilities

Maintain accurate and confidential patient records using the EMIS clinical system.

Ensure compliance with NHS Wales and Public Health Wales protocols, NICE guidelines, Health and Safety and infection control standards.

Monitor group channels such as Microsoft Teams.

Person Specification

Experience Essential

Demonstrable experience in childhood immunisations, injections, chronic disease management, and wound care. Experience in patient education and health promotion.

Desirable

Experience working in general practice. Experience in working with multilingual or diverse patient populations. Experience in quality improvement initiatives or audits within a primary care setting. Experience working within socioeconomically disadvantaged communities.

Additional criteria Essential

Strong interpersonal and communication skills. Proven ability to work independently and as part of a wider team. Excellent organisational and record-keeping skills.

Desirable

Familiarity with NHS protocols, clinical guidelines, and quality standards. Ability to engage proactively with hard-to-reach patient populations.

Diploma or relevant experience to support in additional clinics such as respiratory, pessaries or HRT.
