Main duties of the job
To manage a caseload of community adult clients with multiple and complex needs, using evidence based/client centred principles to assess, plan, implement and evaluate interventions To support the team/clinical leads in providing clinical and supervisory leadership to Community Nurses, Nursing Associates, support workers and students within own area of responsibility, within the multidisciplinary team. To be jointly responsible with other senior colleagues for the day-to-day planning and delivery of the Community Nursing service To support the team leads and other colleagues with the implementation of policy, protocol, service/practice development and change within own area of responsibility. To continue to demonstrate specialist clinical skills in practice and provide support/advice re service development and the delivery of care, in partnership with service users, carers, staff, managers and other agencies.

Qualifications Essential
oPost registration/graduate diploma or equivalent experience in related work area, to demonstrate advanced level of practice. oFull understanding/application of relevant clinical practice/standards/audit within identified clinical area.
Desirable
oEvidence of project work that has impacted on practice and demonstrates a higher level of communication/planning/change that impacts within own area of work/responsibility. oLeadership training/qualification/experience
Experience Essential
oEvidence of promoting/supporting active user/carer involvement/participation oEvidence of continuing professional development (CPD) relevant to the clinical area at specialist level of care
Desirable
oAbility to effectively Chair meetings. oDemonstrable experience of working in the specific field where the post is held.
