Job summary
Symphony Healthcare Services are looking for an enthusiastic and compassionate Nature-Based Health and Wellbeing Coach to join West Somerset Primary Care Network and lead the delivery of the Healthy Exmoor programme, a partnership with Exmoor National Park Authority.
You will be proactive, curious and collaborative, with the enthusiasm to develop partnerships and explore new ideas.
You will contribute to the continuing development of Healthy Exmoor as a place-based approach to personalised care and Green Social Prescribing, helping to create sustainable pathways between healthcare, nature and community.
This is a non-clinical role. You will receive regular supervision, ongoing training and opportunities for continuing professional development, and will be supported to work safely and effectively within the scope of the role.

This job role is a fixed term contract for 18 months, with the option to work between 30 and 37.5 hours per week.
The pay rate is from £13.27 per hour, DOE.

Main duties of the job
This is a unique opportunity to combine person-centered health and wellbeing coaching with the natural environment to support health.
You will provide personalised 1:1 coaching and facilitate group opportunities, supporting people to identify what matters to them, build confidence and self-management skills, and make positive and sustainable changes to their health and wellbeing.
Working across Exmoor National Park and the wider West Somerset community, you will help people to connect or reconnect with nature and their local communities in ways that support physical health, mental wellbeing, social connection, confidence and independence.
You will work creatively with existing opportunities and partners, while developing new nature-based approaches that respond to the needs and interests of local people.
You will work with a diverse range of people and communities, including those who may experience barriers to good health, social isolation or reduced confidence.
You will build trusting, compassionate and non-judgemental relationships, recognising the strengths and potential within each person and supporting them to become active participants in their own health and wellbeing.

You will deliver the Healthy Exmoor programme through a personalised care approach, supporting individuals to improve their physical, mental and social wellbeing through health coaching and meaningful engagement with nature.
You will also manage and prioritise a diverse caseload, providing person-centered support that empowers individuals to identify what matters to them, build confidence, increase self-management and improve their health and wellbeing outcomes.
You will develop and facilitate nature-based health and wellbeing opportunities that utilise Exmoor National Park and the wider natural environment of West Somerset as assets for improving health, wellbeing and reducing health inequalities.
Person Specification
Qualifications Essential
Willingness to complete a Personalised Care Institute (PCI) accredited Health and Wellbeing Coach training programme if not already qualified.
Desirable
Coaching, counselling or other relevant qualifications demonstrating reflective listening and behaviour change skills. Training in Green Social Prescribing or Nature-Based Health and Wellbeing. Walking Group Leader, Lowland Leader or equivalent outdoor qualification
Experience Desirable
Experience of using coaching approaches, motivational interviewing or other behaviour change techniques. Experience of working within health, social care, community development or voluntary sector settings. Experience of facilitating one-to-one and group interventions. Experience of successful partnership working across statutory, voluntary and community sector organisations. Experience of networking and developing collaborative relationships. Experience of delivering or supporting Green Social Prescribing or nature-based health interventions. Experience of working in rural communities.
Knowledge and Skills Essential
