Job summary
Hospice at Home aims to offer additional support, alongside the community nursing teams, to allow people to stay at home and receive personalised high quality end of life care, as well as supporting those important to them.
When time is short, people should be able to choose the place they want to stay and receive the appropriate care and support. For most people this means being with people they love, in their own surroundings, where they feel more comfortable and peaceful.
Our team members, consisting of Nurses and Healthcare Support Workers, are here to support you, and will work collaboratively with the Community Specialist Palliative Care Team, Consultants and Community Teams in order to support you with what really matters, fulfilling your wish to stay at home.
This role is a Band 3 and a recognised Care Certificate is required. If candidates do not hold one they will start as a Band 2 and be support in achieving the care certificate and moving to the Band 3 within their probationary period.

Person Specification
Demonstrated experience as a HCSW Essential
Minimum 1 years experience as HCSW

A full UK driving license and access to a car
