As an Advanced Nurse Practitioner, you'll work autonomously, lead clinical decision-making, and help shape an innovative service that puts people and communities first.
You'll be part of a supportive, forward-thinking Primary Care Network (PCN) committed to Neighbourhood Health, bringing together primary care, community teams, voluntary sector partners, and social care to improve population health and reduce health inequalities.

Main duties of the job
To work within the Care Home Team to provide patient-focused care. To work in partnership with the Primary Care Network to develop the role and team further. To support practices with ongoing management of care home patients and amending Personalised Care & Support Plans and RESPECT forms. To support care home patients with anticipatory care discussions and planning. To provide acute medical treatment for care home patients with undifferentiated and undiagnosed problems seeking a primary health care service including on-call, telephone triage, face to face consultations, and home visits. To assess and manage patients with undifferentiated and undiagnosed health care needs including investigating, treating, and referring patients as appropriate. Independently prescribe (where applicable to the role) and review appropriate medications in accordance with local policies and evidence based medical practice and work collaboratively with the wider team to support patients and to promote medication adherence. To order investigations, follow-up results and arrange appropriate management including seeking advice where appropriate. To work with a multidisciplinary team including community and voluntary sector to improve care home patient support. Actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person. Support practices with ward rounds and MDTs.
About us
Pontefract & Knottingley PCN comprises four GP practices on the east of the district covering a population of approximately 49,000 registered patients. The practices are Northgate Medical Centre, Friarwood Medical Centre, Stuart Road Medical Centre based in Pontefract and Ash Grove Medical Centre based in Knottingley.
Our aim is to build a team of professionals working collaboratively with our volunteer and community services and social enterprise organisation to provide high quality services to our local population.

Qualifications Essential
Registered Healthcare Practitioner Independent prescribing qualification Completion of Post Graduate Degree qualification in own field, Clinical Practice or equivalent qualification with evidence of academic study Evidence of continuing professional development and application to practice within the last 12 months

Experience Essential
Substantial relevant post registration experience of working as an Advanced Clinical Practitioner or similar Consolidated independent prescribing experience Experience of managing patients with complex needs Experience of using autonomous physical examination and assessment skills in practice Experience of telephone triage Can demonstrate a broad and sound knowledge of complex and common conditions in primary care in both adults and children Experience of working effectively and collaborating within an integrated multi-disciplinary environment of teams and professionals Experience of accurate documenting and recording keeping of clinical information Teaching and mentoring in a clinical setting Leading on policy developments and wider initiatives to include funding and project work in line with area of specialism
