Job summary
Wakefield North Primary Care Network is seeking a compassionate and motivated Mental Health Practitioner to join our established multi-disciplinary team supporting patients across six GP practices. The role involves carrying out mental health assessments and providing high-quality care offering advice, support and signposting to patients experiencing poor mental health. Working collaboratively with the GPs and the wider primary care team, you will play a vital role in strengthening community resilience and delivering coordinated care across our six member practices. You will be required to provide tailored advice and guidance within their area of expertise, always guided by external regulations, professional standards and legal processes. Working directly with patients, their carers and families the role holder will be responsible for identifying requirements of the patient, understand risk factors and anticipate potential issues which may arise.
The post holder should have significant experience working within mental health services and be confident working independently and as part of a multi-disciplinary team. Excellent communication, assessment, care planning and risk management skills are essential, alongside a commitment to delivering compassionate, evidence-based care.

Main duties of the job
Provide compassionate, patient-centered mental health support to individuals experiencing poor mental health within GP practices across the PCN.Undertake mental health assessments, risk assessments, and develop personalised care and intervention plans in partnership with patients and carers.Deliver advice, support, signposting, health education, and recovery-focused interventions through telephone and face-to-face consultations.Monitor patient progress, review care plans, and respond appropriately to changes in mental health needs or crisis situations.Maintain accurate and confidential patient records in line with professional standards, GDPR, and organisational policies.Promote equality, dignity, social inclusion, and positive mental health awareness while helping to reduce stigma associated with mental health conditions.Participate in supervision, training, team meetings, and continued professional development to maintain high standards of clinical practice.
About us
We consist of six GP practices on the Eastern side of Wakefield District. Our practices are Henry Moore Clinic, Riverside Medical Centre, Tieve Tara Medical Centre, Station Lane Medical Centre, St Thomas Road Surgery and Newland Surgery.
You will be part of our growing multi-disciplinary additional roles team, who all work together to support our GP practices and their patients. The roles currently include Care Coordinators, Clinical Pharmacists, Pharmacy Technicians, Paramedics, Health and Wellbeing Coaches, First Contact Physiotherapist, Social Prescribing Link Worker and Nursing staff.

Person Specification
Experience Essential
Significant experience of working within a mental health setting. Can demonstrate skills in assessing and managing people experiencing mental health problems. Experience of supporting/mentoring and supervising colleagues/students/trainees. Knowledge and evidence of using IT systems. Can demonstrate skills in assessing and managing risk in a variety of settings. Proven ability in assessing the needs and strengths of people and their carers. Experience of delivering a range of therapeutic techniques. .Working within an integrated multi-disciplinary environment and co-working with other teams and professions within team meetings, case conferences and professional meetings. Ability to support colleagues with day-to-day caseload management including prioritisation of responsibilities in times of high clinical demand. Ability to promote and develop links with other professionals e.g. practitioners within long term conditions, primary care. Evidence of knowledge and application in practice of national guidance and standards, e.g. NICE, CQC.
Desirable
Experience of working holistically with adults with an emphasis on coexisting mental and physical health needs that impact on wellbeing.

Qualifications Essential
RMN Current NMC registration. Social Worker, Occupational Therapist or current HCPC registration Experience of mentoring or supporting learners or ability to demonstrate the skills required for effective mentoring. Practice Education or Practice Assessor Preparation Certificate. Assessment and care planning skills. Risk assessment and risk management. Relevant post-registration qualifications and/or training.
Knowledge and skills Essential
Working knowledge of recent government legislation e.g. Mental Health Act, Health & Safety, Care Programme Approach, Care Act and Safeguarding. Able to work incorporating principles of Clinical Governance. Proven skills in partnership working with people who use services and their carers. Can evidence awareness of equality and diversity and is able to incorporate this into care planning and the delivery of care. Knowledge of audit processes. Experience of carer services/ issues Excellent written and verbal communication skills
Desirable
Knowledge and skills in specialist areas such as dual diagnosis, peri-natal mental health, working with people experiencing psychosis, working with people with a diagnosis of personality disorder
Training Essential
Assessment and care planning skills. Risk assessment and risk management. Relevant post-registration qualifications and / or training.
Other Essential
Holds a full driving licence, which is valid in the UK, and hold the relevant car insurance to cover travel for business purposes, with access to a car
