About this role

Job summary

An exciting opportunity has arisen to recruit 10 Mental Health Practitioners to our community mental health teams across Manchester. This is a excellent time to join our teams as we move towards a transformed clinical offer in our community services. You will be an integral part of shaping the new community provision along with our service users and carers.

Postholders will offer timely and appropriate interventions to service users with enduring mental health needs in the Manchester area.

Mental Health Practitioners will be working in line with standard operational procedures demonstrating respect of individuality, values, culture and religious diversity of clients and colleagues and thus contributing to the provision of a service sensitive to these needs.

Main duties of the job

To demonstrate high-level communication skills.

Be a full member of the multi-disciplinary team (MDT).

Effectively manage own caseload accepting referrals from agreed agencies in accordance with agreed operational procedures.

Ensure compliance with all Care Programme Approach requirements as per Trust Policy.

To participate in the continuous improvement of the service provided by the CMHT.

Manage a defined caseload of clients of any age within the Community Mental Health Team with complex physical and mental health needs in the community, using evidence based/client centred principles to assess, plan, implement and evaluate interventions. Record all interventions in line with relevant clinical pathways (functional or dementia) as outlined within the CMHT operational procedure
