An exciting opportunity has arisen at Wakefield North Primary Care Network (PCN) for a Clinical Pharmacist to join our friendly and dynamic team of Pharmacists and Pharmacy Technicians, with a Lead Pharmacist (ACP) who will support your training and development and act as a mentor when required.
The PCN has six GP practices in the Eastern side of the Wakefield District serving a population of around 48,700 and we are looking to recruit enthusiastic, forward-thinking pharmacists to work as part of our established team looking after the health and wellbeing of our local population.
Main duties of the job
The post holder will be working within their clinical competencies, providing expertise in medicine management, undertaking face-to-face and telephone medication reviews, managing long-term conditions and providing support to the GP practice staff by dealing with acute prescription requests and medicine reconciliation on transfer of care. All the while addressing both the public health and social care needs of patients registered at our GP practices, which includes our elderly population, those living with multi-comorbidities and patients residing in residential and/or care homes.
You will provide clinical leadership on medicines optimisation and quality improvement, managing some aspects of the Quality and Outcomes Framework as well as taking part in Quality Improvement work whilst ensuring improvement in patient outcomes. The role is pivotal to improving the quality of care and operational efficiencies, therefore requires motivation and passion to deliver an excellent service within our general practices.

Experience Essential
Evidence of working autonomously or as part of a team Medicines management and the principles of Medicines Optimisation Experience of communicating with other healthcare professionals and the general public Demonstrable prioritising of workload and using own initiative to solve problems Evidence of personal leadership to improve the quality of care Demonstrable knowledge of polypharmacy and deprescribing
Desirable
Experience working in a primary care setting / PCN Policy/procedure development for General Practice repeat prescribing systems Understanding and knowledge of healthcare provision in GP practices, QOF, and PCN DES An appreciation of the new NHS landscape including the relationships between individual practices, PCNs and the commissioners Use of SystmOne
Qualifications Essential
Master's degree in pharmacy (MPharm) Registration with the General Pharmaceutical Council (GPhC). Evidence of Continued Professional Development
Desirable
Postgraduate Diploma in Clinical Pharmacy (or equivalent) Independent Prescriber relevant to primary care Completion of the Primary care pharmacy education (CPPE) pathway
