The Community Therapy Team provides a high-quality, responsive rehabilitation service for adults, helping people recover following illness or injury, manage long-term conditions and improve their health and wellbeing.
Based at Tooting Works in the London Borough of Wandsworth, you will join a friendly and supportive multidisciplinary team of Occupational Therapists, Physiotherapists, Speech and Language Therapists, Rehabilitation SupportWorkersand Trusted Assessors, working together to deliver person-centred care.
This is an exciting opportunity to take professional responsibility for assessing,planningand delivering occupational therapy interventions for adults referred to the service. Working closely with service users,carersand partner organisations, you will support people to achieve meaningful rehabilitation goals, maximise independence and enhance their quality of life. You will also have opportunities to contribute to service development, qualityimprovementand service evaluation, helping to shape safe,effectiveand responsive community rehabilitation services.
Main duties of the job
Provide occupational therapy assessment and intervention for adults across home and community settings, promoting independence, wellbeing and participation in meaningful activities Undertake assessments of mobility, postureand seating needs, underappropriate supervision, within patients' homes, residential and nursing care settings, and other community locations asrequired. Develop, implementand review person-centred treatment plans in partnership with service users,familiesand carers to support the achievement of agreed rehabilitation goals. Work collaboratively with members of the multidisciplinary team to deliver coordinated, high-quality rehabilitation and maximise service users' independence. Establish and maintaineffective working relationships with health, socialcareand voluntary sector partners, including General Practitioners, acute providers, District Nursing teams, WheelchairServicesand colleagues across the organisation, tofacilitatetimelyandappropriate referrals. Demonstrate an understanding of the occupational therapy role within integrated community services and contribute positively to multidisciplinary team working. Contribute to service development, audit, researchand quality improvement activities to support the delivery of safe,effectiveand responsive services. Maintain professional competence through active participation in Continuing Professional Development (CPD), supervision, mandatory trainingand reflective practice.

Person Specification
Education/Qualification Essential
Diploma/degree in Occupational Therapy Current HCPC registration
Experience Essential
Relevant work experience in a variety of clinical placements. Evidence of up to date CPD Experience of working as part of an MDT Use of functional assessment and activity analysis.
Desirable
Evidence of previous employment as an Occupational therapist, apprentice, assistant
Skills & Knowledge Essential
Ability to work single-handedly with individuals and groups Good organisational skills, ability to prioritise, manage time/caseload effectively Effective written and oral communication skills. Ability to listen and present information, written and orally in a clear, concise and logical manner. Good IT skills (word processing, and internet) Demonstrates an understanding of clinical governance and its implications for the profession
Desirable
Awareness of the legal responsibilities of the profession.
Key Attributes Essential
Able to work both independently, and with direct and indirect supervision. Willingness to work flexibly at a variety of locations, travel from site to site within service delivery within the Borough of Wandsworth
