Job summary

An exciting opportunity has arisen for a Transition Support Worker to join the CYPS Mindworks Community Hub, working within the dedicated Transition Pathway. This pathway supports young people aged 17-18 as they transition out of Children's Emotional Wellbeing and Mental Health (EWMH) Services into appropriate adult provision or wider community support.

The role is designed to provide additional, personalised support to young people whose future care pathway may be unclear, ensuring they receive the right help at a critical stage in their lives. The post holder will act as a consistent and trusted point of contact, offering support over a period of up to six months.

This is a rewarding role suited to someone passionate about supporting young people through change, advocating for their needs, and delivering person-centred, compassionate care.

Main duties of the job

The Transition Support Worker will be responsible forcase management and coordination of care, working collaboratively with the young person to develop a tailored Transition Care Plan that reflects their individual needs, preferences, and goals. The role includes actively supporting attendance at appointments, assessments, and therapeutic sessions, as well as helping young people understand differences between children's and adult services.

Supporting young people through the difficult period of transitioning out of Children's EWMH Services and into appropriate mental health or community services as they reach 18 years of age.

Key Responsibilities

Approachable, friendly and have the ability to engage and build relationships with young people with complex mental health needs.

Being able to think creatively about how to engage with a young person.

Being a consistent and reliable advocate/voice for a young person during the transition period.

Accompany a young person to appointments, assessments, meetings, and therapy as required.

Be able to develop an understanding of a young persons needs/challenges and signposting to appropriate mental health and community services to support that young person.

Assisting a young person to build support networks to help them maintain their independence in the community.

To liaise with colleagues and other professionals or agencies as required, including attending MDTs, planning meetings, transfer of care meetings etc. Confidence to contribute to MDT discussions around plans for a young person. Having a good understanding and be able to work closely with mental health professionals. To hold an individual caseload under the guidance of a qualified member of staff. Being able to work with families in a way which is helpful to that young person.

Being comfortable lone working with a young person.

Must hold a current driving license and have access to a car for work purposes.

IT skills including ability to use a computer.

Ability to accurately and concisely document notes and risks onto system after sessions.

Ability to conduct risk assessments, identify changes to clinical risk and implement actions to ensure a persons safety.

Knowledge of responsibilities in relation to the Childrens Act.

Knowledge and awareness of Safeguarding procedures and the experience of developing a plan of care to safeguard an adult or child.

Person Specification

Qualifications Essential

Degree or equivalent qualification in a children or adult healthcare related subject. Must hold a current driving license and have access to a car for work purposes

Experience Essential

Experience working in a relevant field such as mental health Experience working with adolescents and/or young adults

A person with lived experience
