The Role:
To deliver a high quality, strengths-based approach to statutory social care service, ensuring that adults and their families and carers: Are able to seek strengths-based solutions, assess, manage and review their own needs and risks, maximise their own independence and self-direct their responses to deliver agreed outcomes Can access personalised care and support to meet their assessed needs and maintain themselves in the community Are protected and safeguarded.

Key Responsibilities:
Evaluate mental health, social circumstances, family dynamics, and risk factors as part of the assessment process Develop individualised care and discharge plans alongside multidisciplinary teams Identify and respond to safeguarding concerns Work within frameworks like the Mental Health Act and Care Act Coordinate with housing, community services, and family support systems Advocate for patients rights, preferences, and needs Empower individuals to be involved in decision-making

Clear, compassionate, and non-judgemental communication Ability to engage people in distress or with complex needs Strong report writing and documentation skills Understanding of trauma and mental illness Ability to analyse complex situations quickly and make sound decisions under pressure Balance safety with autonomy Knowledge of Legislation including Mental Health Act, Care Act Work effectively within multidisciplinary teams

Recognised Social Work Qualification Registered or be in the process of registering with Social Work England Practice in accordance with Social Work England Standards
