Job summary
To work as part of the Neonatal outreach service providing care to neonates in the community following a supported discharge from hospital. to be the link professional for care needs and deliver safe and effective care in the community. to be aware of the Neonatal Network and BAPM standards
Main duties of the job
1. Act as an advocate for infants and their families.
2. Safeguard the safety of patients, parents/ carers and staff at all times, maintaining a safe working environment.
3. Participate in the development and implementation of departmental and corporate guidelines, policies, procedures and care pathways.
4. Adhere to Unit and Corporate guidelines and policies, and ensure junior staff are aware of and perform to these standards.
5. Participate in the risk reporting procedures.
6. Ensure all equipment and safety checks are performed before use, taking action to ensure all necessary equipment is available at all times,
7. Be aware of responsibilities in regard to safe guarding children issues and act accordingly, initiating and attending inter agency meetings as required to ensure the best interests of the infant are represented.
8. Be aware of the complaints procedure, dealing with issues as appropriate or referring complainants to senior staff.
9. Participate in the development of programmes that seek to measure outcomes and quality, including audit and research studies under the guidance, support and direction of senior staff.
10. Be aware of and work according to the Lone Worker Policy.

Job description Job responsibilities
To provide individualized community nursing and family centred care to neonates requiring special care without supervision. This includes infants having home oxygen therapy. To assess the care needs of special care infants through the interpretation of results including pulse oximetry and weight measurements. Continually evaluate the quality and effectiveness of care given, implementing necessary changes in treatment with the Neonatal consultant/community lead nurse. Maintain accurate and up to date documentation of care, observations and changes to treatment. Support and assist parents/carers in establishing breast/NGT/Bottle feeding by ensuring correct and effective techniques, working in conjunction with the dietitian as indicated. Ensure developmental care needs are met. Early identification and assessment to co-ordinate plans of care to ensure patient flow through the service for discharge either into the community setting of transfer to other areas/hospitals/community teams 8. Co-ordinate services with external agencies, such as social services, paediatric outreach teams and health visitors and other Neonatal Community Team/professionals To attend the Neonatal unit to join ward rounds to facilitate ongoing discharge planning to the community. To coordinate complex discharge planning and assess readiness for discharge. Identifying areas of support for families and training needs which can then be disseminated to unit staff for completion.

Education and Qualification Essential
RSCN/RN Qualified in Speciality Neonatal qualification Evidence of continued professional development Preparation for mentorship (ENB 988 or equivalent)
Desirable
Management/Team leading qualification
Knowledge and Experience Essential
Experience of shift coordination in a Neonatal setting in the absence of a sister/charge nurse within the last 3 years Acute paediatric nursing experience of working in a team and independently Evidence as working as part of a MDT
Desirable
Neonatal nursing experience in community settings
Skills and Knowledge Essential
Ability to work confidently and autonomously and remain calm under stress IT skills Flexible approach to working pattern Excellent communication skills
