Job summary
We are looking for an experienced and motivated Community Transformation Manager to join Shropshire Community Health NHS Trust in a key leadership role supporting the delivery of our Community Transformation Programme. Working as part of the Operations Directorate, you will lead and support complex service transformation, neighbourhood development, productivity and cost improvement schemes, working closely with operational divisional leads, corporate services, Primary Care and wider system partners across Shropshire, Telford and Wrekin and Dudley. This is an exciting opportunity to help shape the future of community services, supporting the planned shift of care from acute settings and ensuring care is delivered as locally, effectively and conveniently as possible for patients.
The successful candidate will bring significant programme leadership experience, strong project management skills and a sound understanding of NHS structures, healthcare policy and service development. You will be confident working with senior leaders and a wide range of internal and external stakeholders, using excellent communication, analytical and influencing skills to develop robust plans, manage risks and deliver sustainable change. You will also be an effective people manager, able to motivate and develop teams, work flexibly across multiple sites and contribute to a culture of improvement, collaboration and patient-centred care.
Main duties of the job
To lead community transformation and service improvement projects, working with senior leaders and partner organisations to plan, deliver and monitor programmes that improve services, productivity and neighbourhood-based care. Plan and deliver neighbourhood development, and support allied productivity and cost improvement schemes in conjunction with service leads and corporate support services. Work closely with operational divisional leads to deliver service transformation projects linked to the Community Transformation Programme. To work with partners, including Primary Care, on the implementation of the Community Transformation Programme and the planned shift of care from Acute Settings. The key role will include planning and organisation of a broad range of complex projects and programmes, both new and ongoing, which require the formulation and adjustment of plans. To work in partnership with senior leaders across the operations directorate and corporate services with key links to Planning and Quality divisions.

Training and Qualifications Essential
Educated to Masters level in relevant subject or equivalent level of experience of working at a similar level in a specialist area. Extensive knowledge of specialist areas (Service Transformation and/or Service Development) acquired through post graduate diploma or equivalent experience or training. Programme/Project Management Qualification (Managing Successful Programmes, PRINCE 2 or similar). Significant evidence of continued professional development.
Experience Essential
Significant Programme Leadership experience with an understanding of the background to and aims of current healthcare policy. Leadership experience of working collaboratively with individuals from different professional backgrounds. Experience of leading strategy implementation and advising senior managers on related complex issues. Experience of mobilising ad implementing new service developments.
Knowledge and Skills Essential
An understanding of NHS structures, systems, and processes. An ability to demonstrate the complexity of business including the need to achieve results in difficult financial climate. Ability to challenge ways of working at all levels as part of continuous improvement drive. Demonstrated ability to plan over short, medium, and long-term timeframes and adjust plans and resource requirements accordingly.
