About this role

Job summary

Our Community GP's have the opportunity to apply their experience, knowledge and interest of Highly complex patient care in the patients own home through working with the Team Up Home Visiting Service in North Hardwick and Bolsover PCN

You will clinically lead a team of highly skilled and knowledgeable Advanced Clinicians to deliver complex holistic patient care. You will also support the Service Clinical Leads and Operational Lead in developing and applying service improvements.

You will also provide support to our trainees, helping them achieve their academic goals.

Main duties of the job

Provide day-to-day clinical leadership for the multi-disciplinary and multi-agency service to deliver effective, efficient and high-quality Primary Care services to Housebound Patients.

Mentorship, guidance and support to clinical decision making throughout the whole extended team.

Medical assessment and management on behalf of and in conjunction with members of the team.

Senior clinical triage for the Acute Home Visiting service, to determine the urgency and type of response needed, according to clinical need.

Provide education and mentorship to members of the team, especially those in advanced or extended practice roles, including trainees.

Person Specification

Education, Qualifications & Training Essential

A Doctor/Consultant on the relevant speciality register Current registration with the GMC

Desirable

Royal College Membership Mentorship and Supervision Qualification or equivalent experience

Experience and Knowledge Essential

Experience of leading a team of senior clincians Experience of working with multidisciplinary teams Experience of working with patients living with frailty, or a willingness to develop skills in this area Experience of education and mentorship of senior clinical colleagues
