Job summary Key Responsibilities: 1. Promoting effective self-management of patients and/or providing professional intervention for community management in order to present unnecessary hospital admissions, improve quality of life and to actively identify patients who would benefit from case management. 2. This role will include leading the organisation and management of the service and resources, caseload management, benchmarking, clinical audit, the provision of respiratory education and the development of teamwork by identification of common and individual aims. 3. The post holder will make a contribution to both patient and professional education related to the management of respiratory disease in Waltham Forest. 4. The post holder will provide advice and information for the development of the respiratory service in Waltham Forest 5. The post holder will hold caseload and carry out home visits or clinics as appropriate
Main duties of the job Clinical Skills 1. To undertake the comprehensive assessment of patients with respiratory disease , particularly Asthma patients including those with complex presentation and those reaching end of life, using investigative and analytical skills, and to formulate individualised management and treatment plans, using clinical reasoning and utilising a wide range of treatment skills and options to formulate a specialised programme of care. 2. To undertake a holistic, unified assessment process in partnership with all agencies, to ensure that acute admission is prevented where possible and that this approach identifies and delivers services to meet and sustain patients needs. 3. To interpret and analyse clinical and non-clinical facts to form accurate diagnosis and prognosis for patients with complex respiratory disease, to recommend the best course of intervention and to develop comprehensive management plans. 4. To monitor patients with Respiratory Disease, evaluate and modify treatment in order to measure progress and ensure effectiveness of intervention working with the appropriate health and social care professional, ensuring that a seamless approach to care is maintained across all agencies 5. To keep accurate up to date records of patients defined within the case finding process and communicate timely and informative written reports and summaries for other agencies e.g. GPs, Consultants, MDTs and Social Services.

Band 7 Clinical Respiratory Nurse
An exciting opportunity has arisen for a Band 7 Respiratory Practitioner to work with our Respiratory Nursing service in the borough of Waltham Forest
We are looking for an enthusiastic and motivated individual, with excellent communication skills, who is able to work autonomously, as part of the team and to be an integral member of the wider Respiratory teams in supporting people managing the complexities of respiratory management.
The postholder will be involved in facilitating in all aspects of respiratory care , having a sound knowledge of the principles of respiratory/COPD management and care and a keen interest in professional development & advancing with technology. Therefore, experience of teaching and good IT skills is essential.
The post holder will be responsible for providing; supporting and developing the delivery of high quality; evidence based Respiratory clinical practice throughout the local community, in health centre clinics and patients’ homes. The successful applicant will be required to work closely with the lead Respiratory consultant, GPs, nursing colleagues, Physiotherapists and other members of the multidisciplinary team.
The ability to travel effectively across the boroughs is necessary, including home visits

Main duties of the job
1. To further develop Waltham Forest Respiratory services in partnership with Key Stakeholders.
2. To provide high-quality leadership qualities for the Respiratory team by having and implementing organisational knowledge relating to trust protocols and procedures and supporting all to adhere to them.
3. Support the development of high - quality respiratory care throughout the respiratory care pathways, supporting others to do the same through integrated and collaborative working.
4. The respiratory care/education will be delivered in local NHS or community venues, in the patient’s own home or residence, as individual or group sessions as appropriate.
5. To be responsible for own caseload management
6. To participate in the trusts appraisal and supervision system and support others to do the same.
7. To keep accurate and up to date patient records in accordance with local trust, professional body, and legal requirements
8. To participate in and ensure effective risk management behaviours across the WF Respiratory Service by accident/ incident reporting, assessing and controlling risk and ensuring residual risks are added to the trust register.
9. To line manage, supervise and appraise junior members of staff and support the development of staff under direct supervision.
Working for your organisation
Starting with NELFT NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office .
