The Role
This is not a traditional community ACP post. You will make autonomous, high-level clinical decisions within your scope of practice, confidently managing risk to keep patients safely at home wherever appropriate.
This role spans:
Hospital at Home & Digital Ward - clinical responsibility for patients safely managed at home Urgent Outreach/ Crisis Response - undertaking assessments in patients' homes, care homes and community settings Clinical Streaming & ED Interface - working with acute physicians and ED teams to safely redirect and manage patients Ambulance Triage & Avoidance - identifying patients suitable for community-based assessment to prevent unnecessary conveyance

You will:
Undertake complex clinical decision-making across community, intermediate and acute care interfaces. Deliver care within the Clinical Streaming Hub, Hospital at Home, Digital Ward, Urgent Outreach/Crisis Response and associated urgent care services. Assess and manage patients within their own homes, care homes, community settings,clinical environments. Identify patients suitable for admission avoidance pathways and safely support care closer to home. Work collaboratively with Primary Care, ED, Acute Medicine, Welsh Ambulance Services, community teams to ensure patients receive care in the right place and time. Provide senior clinical advice, leadership and support to multidisciplinary colleagues. Contribute to service development, pathway redesign, quality improvement and innovation initiatives. Support education, supervision and professional development of colleagues and learners. Maintain advanced clinical competence, professional registration and prescribing practice in line with national and local requirements

Within Ceredigion the role also includes rotation within the Same Day Urgent Care (SDUC) assessing, diagnosing, treating, and discharging patients with undifferentiated presentations who attend the current SDUC unit in Cardigan, providing key leadership, clinical advice, and navigation within this service

As an Advanced Clinical Practitioner within the Clinical Streaming Hub and Integrated Urgent Community Care Service, you will provide autonomous advanced assessment, diagnosis, treatment and management of patients presenting with urgent and unscheduled care needs.
The role spans a number of integrated services and may include:
Clinical assessment, diagnosis and treatment of patients presenting with undifferentiated and complex urgent care presentations. Clinical decision-making within the Regional Clinical Streaming Hub, supporting patients to access the right care, at the right time, in the right place. Managing patients within Hospital at Home and Digital Ward pathways, including ongoing clinical review and risk management. Undertaking urgent community assessments within patients homes, care homes and community settings as part of Outreach and Crisis Response services. Supporting ambulance triage and admission avoidance pathways by identifying patients suitable for community-based assessment and management. Working collaboratively with Primary Care, Acute Medicine, Emergency Departments, Welsh Ambulance Services and community teams to coordinate patient care. Providing senior clinical advice and leadership to multidisciplinary colleagues. Supporting service development, pathway redesign, quality improvement and innovation across urgent and emergency care services. Contributing to education, mentorship and professional development of colleagues and learners. This post is advertised as a Band 8a and ideally candidates for this role must have achieved an advance practice / relevant masters course including non-medical prescribing (if applicable). However, applicants who do not currently hold the qualification for the Band 8a role are encouraged to apply, and if successful, will need to attain the qualification within 3 years and will be appointed under the All Wales Annex 21 rules.
This means they will be supported by a development plan. (During the first year payment will be 65% of the top rate of band 8a pay, the second year 70% and the third year 75%). Following successful completion of advance practice / relevant masters course including Non-medical prescribing the post holder will then be paid at Band 8a (minimum point on the scale). Support and supervision will be provided in order to develop the successful applicant to the Band 8a role. Failure to achieve the qualification and required competencies in the designated timeframe could result in re-deployment.
