Main duties of the job
In this varied and fast-paced role, you will play a key part in helping patients access the care they need. You will:
Support patients with long-term conditions by inviting them for reviews, tests, and appointments Help manage repeat prescriptions and respond to routine medication queries Contact patients by phone, text, or email to arrange follow-ups and provide updates Keep patient records accurate and up to date using the clinical system Work closely with pharmacy and clinical teams to keep services running smoothly
You wont be expected to make clinical decisions full training and clear guidance will be provided, making this an ideal stepping stone into healthcare.

The Care Coordinator plays a key role in supporting the delivery of safe, effective, and accessible primary care services at scale.
The post holder will support the Pharmacy, LTC and Prescription teams by undertaking high volume, protocol driven coordination and administrative activities, ensuring medicines related and long term condition work is completed efficiently and safely.
This role contributes directly to:
improving patient access and responsivenesssupporting delivery of QOF and population health programmesenabling clinical pharmacists and clinicians to focus on funded clinical activitysupporting medicines optimisation and prescribing quality initiatives aligned to ICB priorities.
The post holder will not undertake clinical decision making but will work within clear protocols, supporting care coordination and patient pathways.
