Job summary
As a Community Care Officer within our newly established Mental Health Social Work Team, you will play a vital role in supporting adults with care and support needs to live fulfilling, independent lives within their communities. This is an opportunity to be part of a developing service, shaping how we respond to mental health needs through a person-centred, strengths-based approach.
You will manage a varied caseload of both non-complex and complex cases, working alongside individuals who may be experiencing a range of mental health challenges. Through building trusted relationships, you will support people to identify what matters most to them and work towards outcomes that promote recovery, independence, and wellbeing.
Using your knowledge of the Care Act, you will complete Adult Needs Assessments and Reviews, ensuring that support is responsive, proportionate, and focused on enabling people to build on their strengths. You will work collaboratively with individuals, families, carers, and partner agencies to coordinate effective, holistic support.
A key part of this role is empowering people to move forward in their liveswhether thats reconnecting with their community, accessing employment or meaningful activity, maintaining stable accommodation, or developing the confidence and skills to manage their own wellbeing. You will take a proactive approach to identifying and reducing barriers, enabling individuals to achieve their personal goals and maximise their independence.
Main duties of the job
1 To work within the policies, practices of the Adult Care and Community Wellbeing Directorate to prescribed quality practice standards and to participate in learning and development as required to ensure Continuous Professional Development.
2 Hold and manage a caseload of service users with less complex needs which will include the identification of service users' needs and the provision of information and advice.
3 Provide support to Qualified Social Workers / therapists / nurses as required in line with the service roles and responsibilities.
4 Contribute, co-operate and work in partnership with other agencies to ensure people are safe and protected and to maximize the outcomes for people.
5 To provide a range of equipment, arrange minor adaptation work and to teach alternative techniques to overcome the effects of disability on independent living where appropriate.
6 Understand the value and usefulness of performance information and uses data to monitor peoples progress and the teams overall performance.
7 Participate in multi-agency meetings. Share written / verbal information to both internal and external parties in line with Directorate policies and procedures and legal requirements.
8 To undertake assessment of needs including any non-social care requirements such as health care or housing to enable planning, design and recording care packages in agreement with people , carers and other agencies within available resources and budgetary constraints.

Core office hours: 0845-1715 Monday-Thursday 0845-1645 Friday
Hybrid working: a combination of home and office-based working, with office attendance as required
Travel is essential; mileage is reimbursed for journeys from the office to appointments
