Job summary

We are recruiting a Band 7 Team Lead to lead a newly established Community Rehabilitation Team (CRT) within the Mid and North Hampshire Division.

This is an exciting opportunity to shape and lead a community-based rehabilitation service, supporting adults with complex mental health needs to achieve recovery and independence in the least restrictive environment.

You will provide clinical and operational leadership, ensuring the delivery of high-quality, recovery-focused care across multiple localities, while driving service development and integration with system partners.

The service focuses on functional recovery and independence, delivering flexible, community-based interventions across multiple localities.

As Team Leader, key elements of the role include:

Providing leadership to a multidisciplinary team delivering rehabilitation interventions Ensuring consistent delivery of care aligned to evidence-based practice and NICE guidance Overseeing service performance, including waiting times, caseloads and outcomes Supporting smooth transitions between inpatient and community services Leading collaboration across system partners to improve pathways and integration Driving quality improvement, audit and service evaluation activityMaintaining oversight of safeguarding and clinical risk within the service

Main duties of the job

The post holder will:

Provide clinical and operational leadership for the Community Rehabilitation Team across Mid & North localities Lead the delivery of a NICE-compliant rehabilitation service for individuals with complex psychosis Oversee team performance, ensuring high standards of quality, safety and access Maintain a reduced but complex caseload, supporting service users with significant rehabilitation needs Coordinate multidisciplinary working, ensuring effective care planning and risk management Support the development of integrated pathways with primary care, acute services, social care and VCSE partners Lead on demand, capacity and flow, ensuring effective use of workforce and resources Oversee referral management and decision-making processes Contribute to service development, transformation and improvement programmesKey skills required:

Strong leadership and decision-making skills Ability to manage complex clinical and operational demands Excellent communication and partnership working skills Strong understanding of recovery-focused rehabilitation and complex psychosis Ability to analyse data and use performance information to drive improvement
