About this role

Job summary

We are recruiting a Band 6 Occupational Therapist to join a newly established Community Rehabilitation Team (CRT) within the Mid and North Hampshire Division.

This is an exciting opportunity to help deliver a new, community-based rehabilitation model, supporting adults with complex mental health needs to develop independence, improve daily living skills and sustain recovery in the least restrictive environment.

You will play a central role in delivering functional, recovery-focused interventions, working as part of a multidisciplinary team to support individuals to live safely and meaningfully in the community. You will work closely with the CMHT providing assessment and intervention.

Main duties of the job

The post holder will:

Deliver specialist occupational therapy assessments and interventions for individuals with complex mental health needsDevelop and implement goal-based rehabilitation plans, focusing on daily living skills, routines and independenceProvide practical, skills-based interventions (e.g. self-care, meal preparation, budgeting, use of community resources)Contribute to risk assessment and therapeutic risk-taking within MDT decision-making, supporting the CMHTDevelop and facilitate group programmes to support recovery and social inclusionSupport service users to access education, employment and meaningful activityWork collaboratively with health, social care, housing and voluntary sector partnersSupervise Occupational Therapy assistant and other Occupational therapistsKey skills required:

Strong clinical reasoning and assessment skillsAbility to deliver functional rehabilitation interventions in the communitySkills in engagement, motivation and therapeutic communicationAbility to work autonomously and within an MDTKnowledge of risk assessment and recovery-based practice

Degree or Diploma in Occupational TherapyHCPC registration
