Job summary

We are recruiting a Band 3 Mental Health Support Worker to join a newly established Community Rehabilitation Team (CRT) within Mid and North Hampshire Divison.

This is an exciting opportunity to be part of a new, community-based rehabilitation service, supporting people with complex mental health needs to live independently, develop their recovery and maintain stability within the community.

You will provide direct, person-centred support to individuals, working alongside a multidisciplinary team to help people build skills, confidence and social networks.

The service focuses on helping people to:

Maintain their independence

Develop daily living skills

Build confidence and social inclusion

Sustain living in their own home or supported accommodation

The team works using an assertive outreach model, delivering care in people's homes and community settings.

As a Mental Health Support Worker, you will:

Work directly with service users to support recovery and wellbeing

Deliver structured interventions and activities as part of care plans

Support individuals to manage their social and practical needs

Assist practitioners in assessment, care planning and review processes

Build supportive relationships to encourage engagement and independence

Contribute to a safe, responsive and recovery-focused service

This is a community-based role, involving travel, lone working and supporting people in a range of settings.

Main duties of the job

The post holder will:

*Provide practical and emotional support to individuals with complex mental health needs

*Work with service users to promote recovery and independence in the community

*Support individuals to identify their goals and needs, contributing to care planning and reviews

*Assist in delivering care plans and rehabilitation activities under the guidance of qualified practitioners

*Support access to community services, social activities and support networks

*Provide 1:1 support and telephone contact, escalating concerns appropriately

*Monitor and report changes in mental health and risk

*Maintain accurate and timely records

Key skills required:

*Strong communication and interpersonal skills

*Ability to engage and build relationships with people who may be distressed or reluctant to engage

*Ability to work independently and as part of a team

*Calm, professional approach in challenging or crisis situations

*Commitment to recovery-focused, person-centred care
