Job summary
We are seeking passionate and forward-thinking Advanced Clinical Pharmacists to join our Clinical Streaming Hubs in the following areas;
1. Eastgate (Llanelli),
2. Porth Preseli (Withybush General Hospital)
3. Cardigan Integrated Care Centre.
We have 1 available post in each of the above 3 sites.
You will be a key member of the Intermediate Care team, working alongside MDT colleagues to support community services. This role offers the chance to apply advanced clinical skills and make a meaningful impact on patient care.
You will deliver high-quality, patient-focused care through consultations, diagnostic assessment (within scope), and comprehensive medication reviews from MDT referrals and home visits. You will also provide expert advice and support complex prescribing decisions.
Applicants must work autonomously, prioritise workload effectively, and commit to ongoing professional development. Strong clinical and communication skills are essential.
An independent prescribing qualification is essential, with opportunities to develop specialist clinical expertise aligned to service priorities.
Main duties of the job
As part of a dynamic and forward-thinking team you will:
Provide expert clinical pharmacy support tothe Clinical Streaming Hub
ProvideComprehensive medication reviews for patients on the caseload(face to face, telephone or in a patient's home) and prescribe/deprescribeappropriately within scope of practice.
Work closely with multi-disciplinary teams, including GP practices and community pharmaciesto ensure safe and effective medicines use
Support antimicrobial stewardship and medicines optimisation initiatives
Provide education and trainingon medications to other clinical staff across theClinical Streaming Hubteam, as well asproviding education and trainingby supervisingand mentoring junior pharmacists, pharmacy technicians, pharmacy students.
Maintainaccurateand thorough records of all patient interactions, usingappropriate clinicaltemplates and coding in line withserviceprotocols.
Leading onclinical auditof medicines prescribed in the serviceandtaking a proactive role inquality improvement initiatives, implementing recommendations whereappropriate toenhance patient care.
Provide patients with clear, independent informationregardingtheir medications and any changes to their prescriptions, referring to other healthcare professionals foradditionalsupport when necessary.

Qualifications and Knowledge Essential
Master's degree in Pharmacy Registered Pharmacist with GPhC. Post-graduate diploma/MSc in clinical pharmacy or equivalent qualification or experience. Independent Prescriber

Experience Essential
