Job SummaryShevington Surgery is seeking an enthusiastic, experienced and qualified Advanced Clinical Practitioner (ACP) to join our highly regarded multidisciplinary team on a full-time basis (37.5 hours per week).
We are an established, forward-thinking GP practice serving approximately 13,500 patients in a semi-rural area of Wigan. The practice is proud to hold an Outstanding CQC rating and is committed to delivering high-quality, patient-centred care.
This is an excellent opportunity for an ACP who is passionate about primary care, clinical excellence and service development. The successful candidate will work autonomously within their scope of practice, providing assessment, diagnosis, treatment and management of patients presenting with a wide range of acute and chronic conditions.

The successful applicant will:
Provide advanced clinical assessment, diagnosis and treatment for patients presenting with undifferentiated and complex conditions.
Independently manage a clinical caseload within primary care.
Prescribe safely and effectively in accordance with professional and regulatory standards.
Request, interpret and act upon investigations and diagnostic tests.
Refer patients appropriately to secondary care and community services.
Work collaboratively with GPs and the wider multidisciplinary team to deliver high-quality patient care.
Contribute to service improvement, clinical governance and quality initiatives.
Support the development of innovative ways of working within the practice and Primary Care Network (PCN).

Job PurposeThe Advanced Clinical Practitioner (ACP) will work autonomously and collaboratively within the multidisciplinary team to provide high-quality, patient-centred care to the practice population.
The post holder will undertake advanced clinical assessment, diagnosis, treatment and management of patients presenting with acute, chronic and complex conditions. They will demonstrate advanced clinical decision-making skills and act within their professional scope of practice whilst supporting service development and quality improvement across the practice.
The ACP will contribute to the delivery of safe, effective and responsive primary care services and support the ongoing development of multidisciplinary working within the practice and Primary Care Network (PCN).
Key ResponsibilitiesClinical Practice
Undertake advanced clinical assessment of patients presenting with undifferentiated and undiagnosed conditions.
Formulate differential diagnoses and develop appropriate management plans.
Order, interpret and act upon diagnostic investigations.
Independently prescribe medications within competence and professional regulations.
Diagnose and manage acute and chronic illnesses.
Provide evidence-based treatment and advice to patients.
Make appropriate referrals to secondary care, community services and other healthcare professionals.
Undertake home visits where appropriate.
Provide health promotion and preventative healthcare interventions.
Maintain accurate, contemporaneous and legally compliant clinical records.
Leadership and Service Development
Contribute to the development and implementation of clinical pathways and practice policies.
Participate in clinical governance activities and quality improvement projects.
Support innovation and service redesign within the practice.
Promote best practice and evidence-based care.
Act as a role model for clinical excellence and professionalism.
Multidisciplinary Team Working
Work closely with GPs, nurses, pharmacists, physician associates and administrative teams.
Liaise effectively with external agencies and healthcare providers.
Support integrated working with PCN colleagues including physiotherapists, paramedics, mental health practitioners, pharmacists, social prescribers and community link workers.
Participate in multidisciplinary meetings as required.
Education and Professional Development
Maintain professional registration and comply with continuing professional development requirements.
Participate in mandatory training programmes.
Contribute to the education and development of colleagues and students where appropriate.
Maintain awareness of current clinical guidance and national policy developments.
Clinical Governance
Adhere to all practice policies and procedures.
Work within professional codes of conduct and scope of practice.
Identify and report significant events, incidents and risks.
Participate in clinical audit and quality assurance activities.
Maintain confidentiality and comply with data protection legislation.
Person Specification
Qualifications Essential
Registered healthcare professional with current registration (NMC, HCPC or equivalent). MSc Advanced Clinical Practice or equivalent recognised qualification. Independent Prescriber qualification. Evidence of ongoing professional development.
Knowledge and Skills Essential
Advanced history taking and clinical assessment skills. Advanced clinical decision-making abilities. Ability to diagnose and manage undifferentiated presentations. Excellent communication and interpersonal skills. IT literacy and experience using clinical systems. Understanding of clinical governance and quality improvement.

Previous experience working in General Practice. Experience using EMIS Web. Experience of service development or quality improvement projects. Experience supervising or mentoring healthcare professionals.
