Job summary
We are currently seeking an experienced Support Worker, at Band 4 to work via a Bank Contract, based in Watford for our Community Adult Mental Health Team. You will be joining our Temporary Staffing Team within HPFT - Hertfordshire Partnership University NHS Foundation Trust.
You will work as part of a small team within a wider multidisciplinary team that provides mental health services to adults. The role involves delivering practical and emotional support in a holistic manner, assisting with the assessment of needs and the provision of social care services.
You will work closely with the Flexible Assertive Community Treatment (FACT) Clinical Lead and other senior staff to ensure coordinated and effective care delivery. Additionally, work within the Care Programme Approach (CPA) and Care Management processes, maintaining a strong focus on recovery principles and supporting service users to achieve their individual goals and improve their overall wellbeing.
HPFT provides services for service users with mental health, learning disability and children's and adolescent mental health problems. Applicants should have some experience of working in a healthcare setting.HPFT has a range of healthcare settings across the County and therefore access to a car would be beneficial unless you have a disability as defined by the Equality Act 2010 which prevents you from driving
Main duties of the job
The job holder will provide support, dedicated time, and encouragement to service users, promoting their recovery and helping them maintain meaningful roles within the wider community and work closely with the FACT Clinical Lead to implement recovery-focused care plans for an allocated caseload of service users, ensuring that they engage positively with their agreed care plans and access appropriate services on a regular and consistent basis.
A key aspect of the role is promoting independent living and empowering service users to achieve their personal goals within the community and to contribute to developing strong therapeutic relationships based on attentiveness, sensitivity, understanding, compassion, and honesty, enabling effective engagement and support. Additionally, assist in developing plans that provide practical support to service users and their carers, helping to maintain dignity, independence, and an improved quality of life.

Job description Job responsibilities
Be responsible for reviewing information available to staff and service users for its quality and relevance.
Help to identify early signs of relapse by monitoring the service users progress, level of functioning and mental state and alert the appropriate staff involved in the service users care.
Report regularly to care co-ordinators and appropriate key workers.
Maintain accurate records as required by existing procedures, entering appropriate details on the service users case notes / electronic patient records as necessary.
Ensure the service user understands and has a clear pathway of care across sector / agency boundaries with key contact points / named individuals.
Participate in and be responsible for appropriate elements of staff development as part of STaR Team meetings as and when required.

Qualifications / equivalent experience: Essential
