About this role

Job summary

We are currently seeking an experienced Community Mental Health Nurse, at Band 6 to work via a Bank Contract, based in Watford for our Community Adult Mental Health Team. You will be joining our Temporary Staffing Team within HPFT - Hertfordshire Partnership University NHS Foundation Trust.

The post holder is required to take responsibility for the assessment of care needs, the development, implementation and evaluation of programs of care, and the setting of standards of care and management for a defined caseload of people with complex mental health and or social care needs. The Community Mental Health Nurse will be an active member of the multi-disciplinary team and will work collaboratively with internal and external partners and agents.

The post holder will carry out supervision, development and teaching of junior staff and/or students as delegated including mentorship and preceptorship roles.

HPFT provides services for service users with mental health, learning disability and children's and adolescent mental health problems. Applicants should have some experience of working in a healthcare setting.HPFT has a range of healthcare settings across the County and therefore access to a car would be beneficial unless you have a disability as defined by the Equality Act 2010 which prevents you from driving

Main duties of the job

You will manage a patient caseload, identified and agreed in consultation with medical, nursing and other professions in the service, and carry out specialist mental health care assessments. You will develop, implement, evaluate and document specialist care. This will include frequent requirement for intense concentration.

The post holder will use a variety of clinical skills appropriate to the needs of the individual and the clinical setting in which they are seen and provide ongoing interventions for people where assessment indicates this to be relevant to their needs.

You will be responsible for the administration, carriage and storage of medication in accordance with relevant Trust policies, and to ensure safe disposal of equipment used in the Community e.g. syringes, needles, in accordance with Trust policies.

Job description Job responsibilities

Deliver care that is person centred and recovery focused, reflecting current best practice and challenge practice that may be detrimental to older people receiving high quality care.

Be aware of the physical needs of people with mental health problems and refer physical conditions on as appropriate.

Use research findings in formulating nursing care plans and develop awareness of the significance of nursing research and wider evidence-based practice in community mental health care.

Demonstrate an awareness of the needs of carers of service users with mental health problems and develop strategies to meet those needs, e.g. self-help groups, respite care, Carers Act.

Participate in the Trusts adverse incidents reporting systems and comply with the Trusts procedure and techniques for managing risks.

The post holder is based within the community mental health service and will be expected to visit people within their own homes, or a variety of public places or voluntary organisations within the community and throughout the Trust.

Person Specification

QUALIFICATIONS/ EDUCATION/TRAINING Essential

NMC Professional Registration RMN or Dip / BSc (Hons) in Mental Health Nursing

PREVIOUS EXPERIENCE Essential

At least 1 years post registration experience at Band 5 in a relevant mental health setting or if newly qualified will participate in the Trusts Preceptorship Scheme.

Desirable

Experience in working with acute mental health problems in a community setting

SKILLS/ KNOWLEDGE/ABILITY Essential

Knowledge of MHA (1983), CPA, Risk Assessment and Section 25 etc. Effective multi professional team communication & team working.
