An exciting opportunity for a Registered Nursing Associate has arisen in the Sudbury Community Health Team. The teamare based in the Health Centre in Sudbury which is a historic market town surrounded by picturesque villages. The SudburyCommunity Health Centre is the base for a wide range of other health professionals and agencies including Adult SocialCare, mental health services and a GP practice so we are able to work together in a fully integrated way.The successful applicant will hold a foundation degree in the field of nursing and will be joining a friendly team ofCommunity Nurses and Therapists. You will be using skills drawn mainly from the professional area of nursing but you mayalso be providing therapy support to patients in the community. You will be visiting the patients in their own home andassessing patients within specified clinical areas as identified by a registered practitioner. You will have the opportunity toprepare for, carry out and monitor assessments and treatments in specified clinical areas, implementing the patient'sindividualised care plan designed in collaboration with the registered nurses in the team
Main duties of the job
On site at the health centre you have access to a range of treatment facilities and may be involved in delivering nursing carein specialist clinics. You will be using a wide range of nursing skills and will need to be confident in your assessment skillsand in your ability to provide safe, effective treatments whilst working in a community setting. Experience of carrying outholistic nursing assessments, supporting the nursing staff to meet a patient's palliative care needs, wound management,diabetic care, catheter care and venepuncture skills will be essential. You will have regular clinical support but you need tobe prepared to be able to work predominantly independently on specified tasks and be able to demonstrate basic clinicalreasoning for treatment progressions. You will have the chance to be fully involved in working with the MDT and integrating with social services, hospice and hospital staff and will need to be committed to ongoing learning and development in the team. The role of the Registered Nursing Associate is to work with the multi professional team and the Registered Nursecontributing to the ongoing assessment of patients and providing and monitoring care in line with care plans which havebeen written and agreed by the Registered Nurse or therapist. You will be accountable for delivering compassionate, personcentred, evidenced based care. You will monitor the condition and health needs of people within your care on a continualbasis in partnership with the multi professional team, recognising and responding to changes in the patients condition andescalating these in a timely way

Education & Qualifications Essential
Nursing Associate Foundation Degree and registered on the NMC register (Nursing Associates)
Experience & Knowledge Essential
Demonstrate knowledge of when to seek advice and escalate to the appropriate professional for expert help and advice. Experience of working in teams under appropriate supervision as part of a multi- disciplinary team. Experience of providing and interpreting complex, sensitive information. Understanding of the scope of the role of the Nursing Associate in the context of the nursing and interdisciplinary team, and how this may contribute to service improvement.
Desirable
Previous relevant experience working in primary and secondary care. Supporting supervision and assessment of students and new starters in the workplace
Skills & Abilities Essential
Ability to organise and prioritise own delegated workload. Good knowledge of working with IT systems.
Personal Qualities Essential
Demonstrate working alone using own inniciative Able to work within a team and to guide others within the scope of the role. Maintain confidentiality of individuals at all times. Able to demonstrate the Trust Values Frequent Travel Essential - You will need to travel, so you must hold a full, current UK driving licence and have access to personal transport.
