Job summary South Rural Primary Care Network is seeking to recruit a PCN patient care coordinator, to be based at Hadleigh Health Centre. This position is offered on a full-time basis.
Main duties of the job
Duties include:
Assist the doctors & other clinicians in the provision of a high quality serviceSupport the practice care home team in the delivery of this service to help manage peoples needs & coordinate the delivery of MDTsWork collaboratively with our care homes to support communication & the administrative work required for the practice to deliver this serviceSupport delivery of QOF, incentive schemes, QIPP, Investment & Impact Fund (IIF), & other quality or cost effectiveness initiativesSupport the delivery of long term conditions clinics working with the clinical team to ensure patients are invited & we reach patients who require additional supportWork collaboratively within the team to manage patients needing medication reviews & support the delivery of this work to ensure all patient groups are reachedSupport of the delivery of our vaccines contractsSupport with the Covid & flu clinicsWork closely with health care professionals to ensure registers are kept up to date & appropriate reviews are completed for palliative, dementia, learning disability & serious mental illnessHelp design efficient templates, protocols, alerts & other IT tools to support safety, patient care, quality record keep keeping & reportingManage cervical cancer recalls & the development of systems to help reach groups where uptake is lowProvide clerical, organisational & information-sharing support to the clinical safeguarding team, which supports the practice to meet statutory requirements

South Rural PCN provides services to 56,000 patients across rural Suffolk and is headed by 2 proactive, supportive and committed Clinical Directors. Our PCN includes the following practices: Bildeston Health Centre, Constable Country Medical Practice (East Bergholt and Capel St Mary), Hadleigh Health Centre, The Holbrook and Shotley Surgeries and Needham Market Country Practice.

Person Specification
If you are a skilled care co-ordinator, with excellent person-centred planning experience and strong local knowledge for accessing key support, then we would love to hear from you. The ideal applicant will have the following skills, experience and qualifications: Essential
Excellent communication skills (written and oral) Flexibility to work outside of core office hours A clear understanding of child protection policy and procedures and commitment to the safeguarding of children and vulnerable adults Experience of working in a primary care setting (desirable) Experience of working in a care home setting (desirable)
