Job summary
We are looking for an administrative individual to join the Diabetes & Endocrinology admin team based at Chase Farm Hospital to oversee the patient pathway from referral to treatment. This involves the implementation of salient NHS targets, regular contact with patients and a keen attention to detail.
Main duties of the job
We are looking for a Patient Navigator to join the Diabetes & Endocrinology admin team who has administrative experience and to work as part of a team within a dedicated speciality/patient group. To ensure that all administrative functions for patients from the time of GP referral to the time of discharge are coordinated in line with specific care pathway instructions to facilitate the smooth running of the patients' journey.
Where a patient has multiple specialist needs, assist the patient in coordinating their pathways to reduce multiple appointments across the Trust. Act as a single point of contact for patients within your remit.
General administrative duties for the department such as filing of all correspondence including patient letters, results, photocopying and distribution of mail, managing mailboxes - identify and action any correspondence suitable for own response - produce relevant correspondence for clinician's reply.
