Job summary

We have an exciting opportunity for a Band 8a Advanced Clinical Practitioner (ACP) committed to delivering high quality care to join our Frailty (Care of the Elderly) team.

The successful candidate will provide comprehensive geriatric assessment (CGA) for frail older adults. This is a great opportunity to be part of a team developing the Frailty provision which includes integrated working between the Emergency Department, wards and primary care & community services.

Main duties of the job

To be part of our team the successful candidate will be an autonomous practitioner ensuring that your practice is underpinned by the four pillars of advanced clinical practice, have recent experience of caring for patients in an acute setting, have excellent leadership and clinical skills, as well as evidence of continuing professional development, have drive, ambition, passion, a flexible approach and dedicated to working in this modern, high quality environment supporting best practice for frailty patients following GIRFT recommendations.

As an experienced ACP, you will provide high quality clinical care for patients, providing a comprehensive advanced clinical assessment, decision making, provision of treatment, prescribing, evaluation of care and referral to other healthcare professionals if required. Whilst working autonomously the ACP will always work within a multi professional team led by a Consultant.

On appointment the successful candidate will be allocated a clinical and educational supervisor, access to eportfolio and will be supported to complete any educational or clinical training for their on-going CPD.

Knowledge & Experience Essential

Advanced life support qualification (ALS/ILS) or willingness to undertake and successfully complete Experience of delivering training / education to others 3 years relevant post registration experience (HEI requirement) Evidence of multi-disciplinary team working

Desirable

In depth knowledge of pathophysiology of common conditions seen in the speciality and their management Experience of working in an extended role with experience of autonomous practice Experience of undertaking service improvement activity Evidence of contribution to audit and research programs Clinical / managerial leadership experience, including line management of staff, e.g. sickness/absence, recruitment/selection Ability to order and interpret appropriate clinical investigations

Qualifications & Training Essential

The candidate must be eligible to apply for and willing to complete a full MSc in Advanced Clinical Practice. Eligibility criteria includes an Honours degree at 2.2 or above in a related health care subject. Alternatively, evidence of completion of pre-entry assessment at HEI or successful level 7 study will also be considered. Registered Nurse with Valid NMC registration or Health Care Professional and registered with HCPC with relevant clinical experience Evidence of relevant continuous professional development

Desirable

Where possible study is funded via the apprenticeship route. If undertaking Apprentice programme, the candidate must be able to demonstrate GCSE English and Maths at C or above or functional skills assessment level 2. If unable to evidence the trainee must complete the functional skills assessment by the end of the Master's Programme (apprenticeship requirement). Post Graduate qualification in Advanced Practice Recognised Leadership Award/qualification or equivalent experience Recognised Teaching & Assessment Qualification or equivalent experience
